人工股骨头置换术治疗老年性股骨粗隆间骨折疗效观察

Efficacy of artificial femoral head replacement on elderly femoral intertrochanteric fracture

目的探讨人工股骨头置换术治疗老年性股骨粗隆间骨折的疗效。方法选择老年性股骨粗隆间骨折患者30例,随机分成两组。观察组15例,采用人工股骨头置换术治疗,对照组15例,采用动力髋螺钉内固定术治疗。比较两组患者术中术后情况以及近期、远期Harris评分。结果观察组手术时间、术中出血量明显高于对照组,但下床负重时间、并发症发生率明显少于对照组(P<0.05);观察组的优良率明显高于对照组(P<0.05);两组患者术后6、12、24个月Harris平均评分差异不明显(P>0.05)。结论人工股骨头置换术治疗老年性股骨粗隆间骨折近期疗效显著,可以缩短卧床时间和恢复时间,改善患者生活质量。

Objective To investigate efficacy of artificial femoral head replacement on elderly femoral intertrochanteric fracture.Methods We selected 30 patients with femoral intertrochanteric fracture,and randomly divided them into 2 groups,observation group 15 cases,artificial femoral head replacement,control group 15 cases,power nail fixation.Intraoperative,postoperative situation and recent,forward Harris scores were compared berween two groups.Results Operative time,blood loss of observation group were significantly higher than those of control group,and time of getting out of bed,the incidence of complications,of observation group were significantly lower than those of control group（P0.05）;Excellent rate of observation group was significantly higher than the control（P0.05）;The average Harris score in the postoperative 6,12 and 24 months of two groups were not significantly different（P0.05）.Conclusion Artificial femoral head replacement on elderly femoral intertrochanteric fracture has significantly recent effect,which can shorten the time in bed and recovery time,improve the quality of life.
