摘要
目的:探讨类风湿性关节炎(rheumatoid arthritis,RA)患者相关血清炎症介质和免疫指标水平的变化及意义。方法:本文分别采用放射免疫分析、酶联免疫法、免疫比浊法检测RA患者血清转化生长因子β1(transforming growth factor-β1,TGF-β1)、基质金属蛋白酶-3(matrixmetalloproteinase-3,MMP-3)、白细胞介素-18(interleukin-18,IL-18)及C-反应蛋白(C-reactive protein,CRP)水平,并分析其水平的变化与临床病情活动程度的关系。结果:IL-18水平缓解期组和活动期组两组均非常显著地高于正常对照组(P<0.01);TGF-β1水平则显著高于正常对照组(P<0.05);MMP-3水平活动期组和缓解期组两组较正常对照组差异具显著性(P<0.01,P<0.05);CRP水平缓解期与正常对照组比较无显著性差异(P>0.05),而活动期组则显著高于正常对照组(P<0.05)。结论:RA患者血清TGF-β1、MMP-3、IL-18和CRP可能参与了RA的发病机制的发生及病情进展过程,该类测定对于了解病情和预后有一定帮助。
Objective To explore the clinical significance of changes on serum TGF-β1 ,IL-18, MMP-3 and CRP levels in pa- tients with rheumatoid arthritis (RA). blothods 80 patients with RA were divided into 2 groups: (1) stable stage group (n = 40), (2) active stage group (n = 40), 40 healthy people served as control groups. The serum TGF-β1 (with RIA), IL-18, MMP-3 (with ELISA) and CRP( with immunoturbiodimetry) levels were determined in 80 patients with RA and 40 eontrols. Results The serum level of IL- 18 was signifieantly higher in stable stage group and active stage group than that in control groups (all, P 〈 0. 01 ). The serum level of TGF-β1 was evidently higher in stable stage group and active stage group than that of eontrol groups (all,P 〈0.05). The serum level of MMP-3 was obviously higher in both the stable stage and active stage group than the controls (P 〈0.05 ,P 〈0.01 ). The serum level of CRP showed a minor changes in stable stage group than the controls ( P 〉 0. 05 ), but the serum level of CRP has risen significantly in aetive stage group than that in eontrol group ( P 〈 0.05 ). Conclusion TGF-β1, IL-18, MMP-3 and CRP might to play important roles in the pathogenesis and the eourse of rheumatoid arthritis. Therefore, determination of which may serve as a predictor to realize and evaluate of RA.
