乙型肝炎病毒经美容手术传播的可能性研究

摘要 目的:探讨乙型肝炎病毒(HBV)经美容手术传播的可能性。方法:在确定美容受术者HBV携带状态和经手术传播感染阈值的基础上,探讨HBV经美容手术传播的可能性。结果:美容受术者HBsAg阳性率(8.9%)显著低于普通手术者(15.7%)(χ2=8.52,P<0.01)。而HBeAg(38.5%)和HBV DNA阳性率(92.3%)却显著高于普通手术者(分别为19.9%与73.7%;χ2=4.54与χ2=4.27;P<0.05)。HBV经针刺损伤传播的感染阈值为105HBV感染剂量/ml。HBsAg和HBeAg双阳性者HBV DNA全部阳性,传染性范围在102~109感染剂量/ml,70%在针刺损伤感染阈值之上;HBsAg阳性、HBeAg阴性者66.7%HBV DNA阳性,传染性范围在0~106HBV感染剂量/ml,13.3%在针刺损伤的感染阈值之上。结论:美容受术者的HBsAg携带率虽低于普通手术患者,但其携带的HBV浓度较后者高,因此经美容手术传播的可能性不容忽视。 Objective To investigate the possibility of HBV spread during aesthetic operation. Methods The ratio of HBV carries among aesthetic operation patients, the HBV spread threshold value and the infectious risk of HBV spread during aesthetic operation were evaluated. Results Of 291 aestheticoperation patients, HBsAg positive ratio (8.9%) was lower than that (15.7%) of ordinary operation patients(x2=8.52, P〈0.01).Positive ratio of HBeAg (38.5%),HBV DNA (92.3%)were higher than that of ordinary operation patients (19.9%,X2=4.54, P〈0.05;73.7%,X2=4.27,P〈0.05).The HBV spread threshold value of needlestick injury was 105 infective dose/ml. In patients with HBsAg positive and HBeAg positive,HBV DNA positive ratio was 100%.Their infectivity unit of HBV spread was 102-109 infective dose/ml and 70% was above the HBV spread threshold value of needlestick injury. Of patients with HBsAg positive and HBeAg negative,HBV DNA positive ratio was 66.7%.Their infectivity unit of HBV spread was 0-106 infective dose/ml,only 13.3% was above the HBV spread threshold value of needlestick injury. Conclusions Although HBsAg positive ratio among aesthetic operation patients was lower than that of ordinary operation patients,the quantity of HBV carries was higher.Therefore, it should be not ignored the infectious risk of HBV spread during aesthetic operation.
