摘要 目的:研究影响正畸医师主观评价面部不对称的软、硬组织因素,为正畸医师临床诊断面部不对称提供参考。方法:5名正畸医师对200张患者的标准正面像进行主观评价,从中选取30例共同判断为面部不对称需要治疗者作为实验组,同时随机选取20例面部对称的患者作为对照组,拍摄头颅定位后前位片。分别测量标准正面像及头颅定位后前位片上左右侧测量项目,计算不对称率,并对测量结果进行统计学分析。结果:硬组织测量中不对称率有统计学意义的项目为:Co-Me、AG-Me、AG-MSR、Co-MSR、Co-AG、AG-H、Co-H,软组织测量中不对称率有统计学意义的项目为:T-MSR、Ch-MSR、Go-MSR、Al-MSR、升支倾角、下颌体倾角,Person相关性分析显示率硬组织下颌角角度(Co-AG-Me)不对称率及下颌角部位水平向不对称率(AG-MSR)高于软组织,ME-MSR与AG-ME,CO-MSR以及CO-AG呈正相关。结论:软、硬组织不对称率的测量有统计学意义项目的确定有助于正畸医师对面部不对称作出诊断。面部软组织有掩饰硬组织畸形的作用。 Objective To determine the soft and hard tissue factors affecting the subjective assessment of facial asymmetry for orthodontists to help diagnosising facial asymmetry. Methods 5 orthodontists examined 200 photographs of patients for subjective assessment.Selected 30 patients who were considered to have severe asymmetry and require treatment for all of the 5 orthodontists and 20 patients who were considered to have moderate asymmetry as comparison.posteroanterior (PA) cephalographs and standardized facial frontal photographs were used for measurements.All of the values were calculated and analyzed. Results Co-Me,AG-Me,AG-MSR,Co-MSR,Co-AG,AG-H, Co-H was statistically significant in hard tissue asymmetry indexes measurements.T-MSR,Ch-MSR, Go-MSR,AI-MSR,body inclination and ramus inclination was statistically significant in soft tissue asymmetry indexes measurements.Person analysis showed that Asymmetry indexes of gonial angle and AG-MSR was higher in hard tissue than soft tissue.ME-MSR was correlation with AG-ME,CO- MSR and CO-AG. Conclusion The determination of soft and hard tissue values who were measured to be statistically significant will help orthodontists for diagnosis of facial asymmetry.skeletal deformities can be hidden by soft tissues.
