摘要
目的:探讨胃癌组织中表皮生长因子受体(EGFR)、人类表皮生长因子受体2(C-erbB-2)、拓扑异构酶Ⅱα(TopoⅡα)和P-糖蛋白(P-gp)的表达及其与临床病理特征的关系。方法:应用免疫组化SP法检测130例胃癌组织中EGFR、C-erbB-2、TopoⅡα和P-gp的表达情况,并结合临床病理特点进行分析。结果:EGFR、C-erbB-2、TopoⅡα和P-gp在胃癌组织中的阳性表达率依次为87.7%(114/130)、53.8%(70/130)、50%(65/130)和33.8%(44/130)。EGFR、C-erbB-2、P-gp在高、中分化胃癌中的阳性表达率明显高于低分化胃癌(P<0.05);C-erbB-2在直径<5cm的胃癌(胃体、胃窦部和幽门部,Ⅲ期和Ⅳ期胃癌)中的阳性表达率明显高于直径≥5cm的胃癌(贲门部及I期和Ⅱ期胃癌),(P<0.05);EGFR在贲门和胃体部胃癌的阳性表达率明显高于胃窦及幽门部胃癌(P<0.05);TopoⅡα在有淋巴结转移的胃癌中表达率明显低于无淋巴结转移的胃癌(P<0.05)。胃癌组织中EGFR和TopoⅡα的表达呈正相关(P<0.05)。结论:EGFR、C-erbB-2、TopoⅡα及P-gp参与胃癌的生长、侵袭和转移过程。它们的联合检测可作为判断胃癌预后、筛选高危转移患者的有效指标,同时也对胃癌的靶向药物治疗有指导意义。
Objective:To detect the expressions of epidermal growth factor receptor (EGFR), human epidermal growth factor receptor - 2 ( C - erbB - 2 ), Topo IIα and P - gp in gastric cancer tissues, and to analyze the relationship among them and the clinicopathologic factors of gastric cancer. Methods:The SP immunohistochemical stain was used to detect EGFR, C -erbB- 2, Topo IIα and P -gp protein expressions in sample of 130 gastric cancer tissues . And their corresponding clinical data were analyzed retrospectively. Results:The expression rates of E GFR, C - erbB -2, Topo IIα and P - gp protein in gastric cancer tissue were 87.7 % (114/130) ,53.8 % (70/130) ,50% (65/130) and 33.8% (44/130). EGFR, C- erbB- 2, P-gp expressions were obviously higher in high and middle differentiation gastric carcinoma than in poorly differentiated gastric carcinoma ( P 〈 0. 05 ) ; An obvious increasing tendency as tumour's diameter 〈5cm,tumor in corpus ventriculi or in sinus ventriculi and pylorus,tomour in stage and IVstage were Showed by the positive expression rates of C - erbB -2 (P 〈0. 05 ). EGFR Ⅲ expression was obviously high in gastric cardia and corpus ventriculi than in sinus ventriculi and pylorus in gastric carcinoma ( P 〈 0.05 ) ; Topot expression was correlated with lymphatic metastasis occurred( P 〈 0.05 ). There was a stable positive correlation between EGFR and Topo ]] ~x expressions in gastric cancer tissue (P 〈 0.05 ). Conclusion: EGFR, C - erbB - 2, Topo I1 ct and P - gp expressions participate the development,invasion and metastasis process of gastric cancer. Combined detection of them can be lookeff as an important symbol for judging the prognosis of gastric cancer and screening the high - risk metastasis patients, and guiding the molecular targeting therapy of gastric cancer.
