82颗磨牙根管治疗失败的体会

Experience of endodontic failure in 82 molars

目的探讨影响磨牙根管治疗失败的因素。方法对76例患者共82颗磨牙根管治疗失败前后的x线牙片进行观察，从根管的长度、形态、数目、钙化、弯曲度方面分析治疗失败的原因并总结经验。结果76例患者的82颗磨牙中，牙龈有瘘道、不适或咬颌痛的患牙56颗（683％），因修复治疗需要重新诊治的16颗（19．5％），因牙体缺损或继发龋等就诊的患牙10颗（12．2％）。在根管治疗失败的82颗磨牙中，62颗（75．6％）欠充填，11颗（13．4％）超充填，5颗（6．1％）根尖渗漏，4颗（4．9％）根管遗漏。结论磨牙根管治疗难度较大，必须提高术者的技术操作水平和临床经验。

Objective To investigate the causes of endodontic failure in molars. Methods The X-ray films of 82 molars in 76 patients before and after unsuccessful treatment were observed. And we analyzed the causes of failure from the length, shape, number, calcification and curvature of roots, and sum up the experi- ences. Results The reasons of endodontic failure in 82 molars included 56 cases （68.3%） with fistulous tract, discomfort and bite jaw pain in gingiva. 16 cases （19,5%） needed repeated diagnosis for recovery treatment. 10 cases （12.2%） of teeth defect or secondary caries. The 82 molars of endodontic failure manifested on missing of the root （62,75.6%）, filling materials extruded （11,13.4%）, apical leakage （5,6.1%） and root canal omissions （4,4.9%）. Conclusion Molars＇ root canal treatment is very difficult. The operators must improve their technical levels and clinical experiences.
