Role of department of pharmacy in use of antibiotics and infection control

摘要 目的分析药剂科在医院抗菌药物使用与感染管理中的地位及作用,为医院抗菌药物的合理使用提供依据。方法随机选取2010年2月-2012年1月共780份病历资料进行研究,将2010年2月-2011年1月病历资料390份设定为非干预组,将2011年2月-2012年1月病历资料390份设定为干预组,观察并对比两组患者抗菌药物用药情况。结果两组抗菌药使用不合理分析,干预组更换药品、联合用药、药物选择、单次剂量计术后用药时间较非干预组显著降低,差异有统计学意义(P<0.05);两组术前用药时间比较差异无统计学意义;两组抗菌药物应用,干预组使用抗菌药物平均费用(1146.57±269.52)元、住院时间(11.41±2.23)d、药物的使用品种(1.67±0.38)种及应用天数(4.78±1.22)d,较非干预组显著下降,差异有统计学意义,医保范围比例显著升高,差异有统计学意义(P<0.05)。结论加强科学监管并进行临床药学实践是保证抗菌药物合理使用的有力措施,提升药剂科服务功能,提高医院感染管理水平。 OBJECTIVE To analyze the role of the department of pharmacy in the use of antibacterial drugs and the infection control so as to provide basis for the reasonable use of antibiotics. METHODS The medical records of totally 780 cases of patients who randomly selected and studied from Feb to Jan 2012, the 390 patients who were treated from Feb 2012 to Jan 2011 were set as the non-intervention group, and the rests of 390 cases who were treated from Feb 2011 to Jan 2012 were selected as the intervention group. The use of the antibiotics between the two groups was observed and compared. RESULTS The analysis of unreasonable use of antibiotics indicated that the antibiotics dressing, combined medication, drug selection, and the time of single dosimeter of the intervention group were significantly decreased as compared with the non-intervention group, the difference was statistically significant(P〈0. 05 ); the difference in the preoperative medication time between the two groups was not statistically significant. As compared with the use of antibiotics between the two group, the mean cost of the antibiotics of the intervention group was(l146. 57±269.52) yuan, the hospitalization duration (11. 41±2.23)d, the antibiotics used (1. 67 ±0. 38) types, and the days of use of antibiotics (4. 78 ± 1. 22) days, which were significantly decreased, the difference was statistically significant, and the proportion of the coverage of medical insurance was significantly improved, with statistical significance (P〈0.05). CONCLUSION To strengthen the scientific supervision and clinical pharmacy practice is the effective way to ensure the rational use of antimicrobial agents and can upgrade the function of the service of pharmacy department and improve the level of control of nosocomial infections.
