摘要
目的探讨提高高位空肠平滑肌肿瘤的诊治水平 .方法回顾性分析 12例距十二指肠悬韧带 15 cm以内空肠平滑肌肿瘤病例 .结果本组 12例中 ,距十二指肠悬韧带小于 5 cm 4例 , 5～ 10 cm 6例 ,大于 10 cm 2例 ,均为腔外型 .术前误诊率为 75 0% ,手术切除率 100% ,其中平滑肌肉瘤 4例 ,平滑肌瘤 8例 .生存 9例 ,死亡 3例 ,生存最长者达 20年 . 5年生存率为 58 3% .结论本病误诊率较高 ,提高诊断水平至关重要 ,出血危及生命者应行剖腹探查术 ,根治性切除预后较好 ,对复发者行再次手术切除肿瘤可提高生存率 .
Objective To study the diagnosis and treatment of smooth muscle tumors of proximal jejunum. Methods Twelve cases with gastrointestinal bleeding due to smooth muscle tumors of proximal jejunum were analyzed retrospectively. Results Among 12 cases,the lesions in 4 cases were located less than 5 cm from Treitz- ligament, 6 cases between 5 and 10 cm and 2 cases more than 10 cm.All cases were exogastric type. Preoperative misdiagnosis rate was 75 0% and resection rate 100% . The 5- year survival rate was 58 3% .Conclusions Preoperative misdiagnosis rate of this disease is high. Those with lethal massive bleeding should undergo laparotomy. Radical surgery for primary tumor is recommended. Early diagnosis and re- excision can improve a long- term survival rate of patients with recurrent tumor.
