摘要
目的 为解决CT导向脑立体定向手术中AC-PC线与定向仪框架成角计算不精确的问题。方法 应用纯数学模型以解析几何法求脑深部结构的定向仪框架空间坐标。结果 从理论上证明,任意角度按装定向仪框架求脑深部结构的定向仪框架空间坐标是可行的,新方法较常规方法更接近金标准(脑室造影法),并具有统计学意义。为简化计算,对新方法作了计算机程序化处理。结论 从理论和实践证明任意角度按装定向仪框架求脑深部结构的定向仪框架空间坐标是可行的。
Objective To facilitate calculating the angle between the AC-PC line and the stereotactic frame in CT guided stereotactic neurosurgery. Method By applying the analytic geometry method, pure mathematical model was used to get 3 dimensions coordinate of the deep brain structure in a coordinate system of the frame. Results The results of this study theoretically proved that it is possible to target the deep brain structures when the stereotactic headframe was at any angle with the AC-PC line during the processes of the neurosurgery. This method has yielded less difference from the golden standard (i.e. ventriculography) than that of the conventional CT guided localization. A computer software has been provided for simplifing the calculation in the applying of this method. Conclusion It was proved that calculating spatial position of the deep brain structure in the headframe coordinate system was possible both theoretically and practically when the frame was at any angle with the AC-PC line.
