摘要
目的探讨双罐串联血浆灌流对血浆百草枯(plasma paraquat,PPQ)的清除效果。方法对比分析双罐串联及单罐血浆灌流对血浆百草枯的清除速度以及在血浆百草枯清至可测定浓度以下所需时间、所用灌流器个数。结果①百草枯的清除速度:第1次灌流时,双罐(16例)及单罐(18例)对百草枯的清除速度为:(14.36±5.72ml/min,10.37±5.67ml/min,P=0.049);第2次双罐(16例)及单罐(18例)的清除速度为:(21.53±7.74ml/min,15.58±8.14ml/min,P=0.037);第3次双罐(8例)及单罐(15例)的清除速度为:(20.23±8.12ml/min,15.54±8.39ml/min,P=0.211);第4次双罐(6例)及单罐(11例)的清除速度为:[19.17(13.33,25.61)ml/min,11.43(6.86,14.74)ml/min,P=0.023]。②双罐及单罐灌流将血浆百草枯浓度清至可测定浓度以下所需时间及所用灌流器个数分别为:(9.00±3.63h,12.67±4.67h,P=0.016)及(6.00±2.42个,4.22±1.56个,P=0.015)。结论与单罐血浆灌流相比,双罐血浆灌流可提高血浆百草枯的清除速度、缩短灌流时间,可能是治疗百草枯中毒的一个有效手段。
Objective To study the clearance of plasma paraquat(PPQ) using plasma perfusion with dual cartridges in series.Methods PPQ clearance rate,time required for plasma perfusion,and the number of cartridges used were analyzed between plasma perfusion with dual cartridges in series and that with single cartridge.Results(a) In patients using plasma perfusion with dual cartridges in series and those with single cartridge,PPQ clearance rate was 14.36±5.72 and 10.37±5.67 ml/min(P=0.049),respectively,during the first plasma perfusion,21.53±7.74 and 15.58±8.14 ml/min(P=0.037),respectively,during the second plasma perfusion,20.23±8.12 and 15.54±8.39 ml/min(P =0.211),respectively,during the third plasma perfusion,and 19.17(13.33,25.61) and 11.43(6.86,14.74) ml/min(P =0.023),respectively,during the fourth plasma perfusion.(b) In patients using plasma perfusion with dual cartridges in series and those with single cartridge,the plasma perfusion period required to decrease PPQ to indiscernible level was 9.00±3.63 and 12.67±4.67h(P =0.016),respectively,and the number of cartridges required to decrease PPQ to indiscernible level was 6.00±2.42 and 4.22±1.56(P =0.015),respectively.Conclusion Plasma perfusion using dual cartridge is an effective method for the treatment of PPQ poisoning,and is more effective than that using single cartridge in the time required for PPQ clearance.
