血清总IgE与25羟维生素D在小儿支气管哮喘病情变化中的意义 被引量:4

摘要 目的探讨血清总IgE与25羟维生素D在小儿支气管哮喘病情变化中的意义。方法随机选取我院2009年1月至2012年10月诊治的支气管哮喘患儿85例,其中缓解组40例,急性发作组45例,另选取40例同期体检健康儿童为对照组。采用放射免疫法检测各组患儿血清总IgE与25羟维生素D水平,并分析两者与患儿病情的相关性。结果与健康对照组比较,哮喘急性发作组和哮喘缓解组血清总IgE明显升高,其中急性发作组水平最高,三组间比较具有统计学差异(P<0.05)。哮喘急性发作组和哮喘缓解组血清25羟维生素D水平均低于健康对照组,具有统计学差异(P<0.05),而急性发作组和哮喘缓解组间比较无统计学差异(P>0.05)。血清总IgE在哮喘急性期轻、中、重组间存在统计学差异,随着病情的加重IgE水平升高,具有正相关性(r=0.714,P<0.05),血清25羟维生素在哮喘急性期三组间无统计学差异(P>0.05)。结论 IgE参与了支气管哮喘患儿急性发作过程,而25羟维生素缺乏可能是引起小儿哮喘发作的一个重要原因。 Objective To study the serum total IgE and 25-(OH) D3 in peripheral blood of asthmatic children and their relationship with the situation of asthma. Methods 85 patients were randomly selected, with 45 cases of acute period asthma, and 40 cases of alleviated period asthma. 40 healthy children were enrolled as the control group. Double-antibody radioimmunoassay was used to detect the levels of serum 25-(OH) D3 and total IgE. The relationship of serum total IgE and 25-(OH) D3 and the situation of asthma was analyzed. Results Compared with the healthy control group, the level of serum total IgE in acute period asthma group and alleviated period asthma group were increased ,and the differences were statistically significant (P〈0.05). Serum 25-(OH) D3 levels decreased significantly in acute period asthma group and alleviated period asthma group compared with those in control group (P〈0.05), but it was insignificantly different in acute period asthma group and alleviated period asthma group (P〉0.05). The levels of serum total IgE in mild, moderate and severe asthmatic patients had significant difference (P〈0.05), and it was positively correlated with the course of childhood asthma (r = 0.714, P〈0.05). Serum 25- (OH) D3 was insignificant correlated with the course of childhood asthma. Conclusions The serum total IgE may play an important role in the pathogenesis of asthma, and the lack of serum 25-(OH) D3 may be a cause of asthma.
