单节段张力加压复位内固定系统治疗单纯性腰椎峡部裂

A single-segment tension compression reduction and fixation system for the treatment of pure lumbar spondylolysis

目的探讨单节段张力加压复位内固定系统治疗单纯性腰椎峡部裂的临床疗效。方法 2009年12月至2012年1月解放军第一八八医院应用单节段张力加压复位内固定系统结合自体骨峡部植骨治疗单纯性腰椎峡部裂患者14例。患者均常规行腰椎X线片、CT和MRI检查。采用Henderson标准评价疗效。结果患者随访3～12个月,随访时间平均10.9个月。按Henderson标准,优9例、良4例、可1例,优良率达13/14。无一例内固定失败及神经症状加重。结论应用单节段张力加压复位内固定系统结合自体骨峡部植骨治疗单纯性腰椎峡部裂具有操作简单、创伤少、安全性高、疗效肯定的优点;病例的合适选择、适应证的把握以及手术操作者的经验是保证临床疗效的关键。

Objective To explore the clinical outcomes of a single-segment tension compression reduction and fixation system for pure lumbar spondylolysis. Methods From December 2009 to January 2012, 14 cases with pure lumbar spondylolysis were treated in the 188th Hospital of PLA using single-segment tension compression reduction and fixation system with autogenous bone grafting in isthmic position. CT, MRI and X-rays were perfomled for all patients. Results All cases were lbllowed tip from 3 to 12 months, with the average of 10.9 months. According to Henderson standard, there were excellent in 9 cases, good in 4 cases, thir in I case, with the excellent and good rate was 13/14. No fixation failure or nerve syndrome aggravation occurred. Conclusion Single- segment tension compression reduction and fixation with autogenous bone grafting for pure lumbar spondylolysis has the advantages of simple procedure, less injury, high safety, and good clinical outcomes. Proper cases chosen, good understanding of indications, and precious experience of surgeon are the key points for the treatment of pure lumbar spondylolysis.
