卡培他滨单药治疗复发转移性胃癌临床分析

Clinical analysis of single Capecitabine in treatment of metastatic gastric cancer

目的观察并探讨临床上使用卡培他滨单药二线治疗复发转移性胃癌的效果及不良反应情况。方法将本院收治的治疗后区域性复发或治疗后广泛转移的50例患者随即分为两组,每组25例,其中观察组患者卡培他滨单药二线治疗,卡培他滨2000mg/(m2·d),d1～8,停药7d;对照组采用多西他赛联合顺铂方案,多西他赛60mg/m2,d1;顺铂25mg/(m2·d),d1～3,两组治疗均21d为1个周期,治疗3～5个周期后,评价两组疗效及不良反应情况。结果观察组有效率为28.00%,对照组有效率28.00%,差异无统计学意义(P>0.05),两组患者毒副作用主要为血液毒性和消化道反应,对照组Ⅲ、Ⅳ级的血液学毒性和肠道反应发生率均明显高于观察组(P>0.05)。结论临床上单独使用卡培他滨口服治疗二线治疗复发转移性胃癌的疗效较好,与联合用药方案疗效相当,但其毒副作用明显较联合方案小,值得临床推广。

Objective To observe and investigate the clinical curative effect and adverse reactions of single Capecitabine in the second-line treatment of metastatic gastric cancer. Methods Fifty patients having posttreatment regional recurrence or widespread metastasis admitted to our hospital were divided into two groups, with 25 patients in each group. The observation group was given single Capecitabine second-line treatment, with Capecitabine 2000 mg/ （m^2·d） from day 1-8 and withdrawal for 7 days. The control group was given docetaxel combined with Cisplatin, with docetaxel 60 mg/m2 at day 1 and Cisplatin 25 mg/（m^2·d） from day 1-3. For both groups, one treatment period consist- ed of 21 days, and after 3-5 periods, the curative effect and adverse reactions of the two groups were evaluated. Re- suits The effective rate was 28.00% in the observation group and 28.00% in the control group, with no statistically significant difference （P 〉 0.05）. The main toxic and side effects of the two groups were hematologic toxicity and gas- trointestinal reactions; the control group was significantly higher than control group in incidence rates of all grade Ⅲ and Ⅳ hematology toxicity and intestinal reactions （P 〉 0.05）. Conclusion The single application of oral Capecitabine shows good curative effect in the second-line treatment of metastatic gastric cancer, whose curative effect is compara- ble to the combination regimen, but its side effects are significantly lower than those of the combination regimen, is worthy of promotion.
