平阳霉素碘油乳剂超选择栓塞治疗肝血管瘤的临床疗效

Clinical effect of Pingyangmycin lipiodol emulsion embolization in hepatic hemangioma

目的探讨平阳霉素碘油乳剂超选择栓塞化疗治疗肝血管瘤(HCH)的临床疗效。方法选取60例HCH患者,随机分为观察组(30例)给予平阳霉素碘油乳剂超选择动脉栓塞化疗,对照组(30例)给予Embosphere微球超选择动脉栓塞化疗,随访1年,分别在治疗前和治疗后6个月、12个月复查肿瘤大小和临床症状改善情况,观察临床疗效和并发症发生情况。结果术前血管瘤直径(8.2±2.3)cm,术后复查CT,瘤体血液供应消失,瘤体缩小,碘油和Embosphere微球沉积良好,术后6个月瘤体直径(5.3±1.4)cm,术后12个月瘤体直径(3.3±1.1)cm,差异有统计学意义(P<0.05)。随访1年,观察组患者治疗的总有效率为86.6%,对照组治疗的总有效率为86.7%,两组比较差异无统计学意义(P>0.05)。两组患者术后临床症状均缓解或消失。术后均没有出现肝坏死、胆囊坏死以及胆管狭窄等严重并发症。结论平阳霉素碘油乳剂超选择栓塞化疗在治疗HCH方面临床疗效确切,安全可靠,值得临床推广和普及。

Objective To observe the clinical effect of pingyangmycin lipiodol emulsion embolization in hepatic hemangioma（HCH）.Methods 60 cases of HCH were collected as the object,were randomly divided into observation group and control group,each had 30 cases.The observation group was treated by pingyangmycin lipiodol emulsion superselective arterial chemoembolization.The control group was treated by Embosphere microspheres superselective arterial chemoembolization.During a follow-up period of 1 year,the tumor size and improvement of symptoms were tested before and after treatment 6 and 12 months.And the clinical effect and complications were observed.Results The hemangioma diameter was（8.2±2.3）cm before treatment,but it reduced after treatment,and the iodine oil and Embosphere microspheres deposition was good.6 months after surgery,the hemangioma diameter was（5.3±1.4）cm.12 months after surgery,the hemangioma diameter was（3.3±1.1）cm,all were obviously reduced withP0.05.The total effective rate of the observation group was 87.8%,it of the control group was 86.7%.There was no significant difference in the effective rate withP0.05.The clinical symptoms were all alleviate or disappear.All cases didn＇t appear severe complications such as hepatic necrosis,gallbladder necrosis and bile duct stricture.Conclusions Pingyangmycin lipiodol emulsion embolization in hepatic hemangioma has accurate clinical curative effect and high safety evaluation,is worth clinical spreading.
