门冬胰岛素对妊娠期糖尿病患者的婴儿心功能影响 被引量:4

摘要 目的探讨门冬胰岛素对妊娠期糖尿病患者婴儿的心功能影响。方法将50例妊娠期糖尿病患者分为观察组(25例)和对照组(25例);观察组患者给予门冬胰岛素,对照组给予生物合成胰岛素;对两组患者所产婴儿行心脏彩超检查,测定心室Tei指数、心室功能;检测室间隔缺损、动脉导管未闭、肺动脉高压及心肌肥厚等情况;同时选取25例健康孕妇所产婴儿为正常组,并观察上述指标作为对比。结果对照组婴儿的左室收缩末期容积高于观察组及正常组,差异有统计学意义(P<0.05);对照组婴儿的二尖瓣E峰与A峰比值、三尖瓣E峰与A峰比较明显低于观察组和正常组,差异有统计学意义(P<0.05);对照组婴儿左心室Tei指数明显低于观察组和正常组,差异有统计学意义(P<0.05);观察组与对照组婴儿心脏异常状况发生率差异无统计学意义(P>0.05)。结论对妊娠期糖尿病患者使用门冬胰岛素,可使其所产婴儿心脏解剖结构及功能性指标更接近正常状态。 Objective To investigate the influence of Insulin Aspart on the infant heart function of patients with gestational diabetes.Methods 50 patients with gestational diabetes were divided into the observation group(25 cases) and the control group(25 cases).The observation group was given Insulin Aspart and the control group was given biosynthesis Insulin.The infants of the two groups were given cardiac color ultrasound in order to measure their ventricular Tei index and ventricular function;the ventricular septal defect,patent ductus arteriosus,pulmonary hypertension,cardiac hypertrophy and other conditions were measured.Meanwhile,25 infants delivered by healthy gravidae were selected as the normal group and the above indicators were observed.Results The control group was higher than the observation group and the normal group in the left ventricular end-systolic volume,with statistically significant differences(P 0.05);the control group was lower than the observation group and the normal group in the ratio of mitral valve E peak to A peak and the ratio of tricuspid valve E peak to A peak,with statistically significant differences(P 0.05);the control group was lower than the observation group and the normal group in the left ventricular Tei index, with statistically significant differences(P 0.05);the observation group and the control group were not significantly different in the incidence of cardiac abnormalities(P 0.05).Conclusion The application of Insulin Aspart in patients with gestational diabetes can bring the infants' cardiac anatomic structure and functional indicators closer to the normal state.
