摘要
目的研究脉博波速度(PWV)与COPD心血管危险因素的相关性。方法应用自动PWV测定仪测定颈-股动脉脉搏波速度(C-FPWV)和颈-桡动脉脉搏波速度(C-RPWV)分别作为评估大动脉缓冲能力和外周中等肌性动脉弹性的指标。对86例COPD患者和64例健康人进行了PWV检测,并测定血清一氧化氮(Nox)、内皮素(ET)。结果 COPD患者C-FPWV、C-RPWV明显高于正常对照组,P<0.001;COPD患者NOX、NOX/ET明显低于正常对照组;COPD患者ET高于正常对照组,P均小于0.001。吸烟指数、年龄、FEV1%Pre是影响C-FPWV的独立因素;NOX、ET、FEV1/FVC%是影响C-RPWV的独立因素。结论 COPD为一种系统性疾病,大动脉弹性的降低为COPD发生心血管疾病的危险因素。
Objective To study the association between pulse wave velocity(PWV) and risk factors of angiocarpy in patients with chronic obstructive pulmonary disease.Methods Automatic pulse wave velocity determinator was applied to examine carotid-femoral PWV(C-FPWV) and carotid-radial PWV(C-RPWV) as the parameters reflecting central elastic large arterial and peripheral muscular medium-sized arterial elasticity respectively.86 COPD patients and 64 healthy people were given PWV detection,and the levels of Nox and ET were determined.Results The value of C-FPWV and C-RPWV was significantly higher in the COPD group than in the control group(P0.001).The levels of NOX and NOX/ET were obviously lower in the COPD group than in the control group,but the level of ET was higher in the COPD group than in the health controls(P0.001).Smoking index,NOX,ET,age,and FEV1% Pre were positively related to C-FPWV independently,while NOX,ET,and FEV1/FVC% were significantly associated with C-RPWV.Conclusion As a systemic disease,the decrease of arterial elasticity is a risk factors of cardiovascular disease in patient with COPD.
