支气管镜肺泡灌洗术联合哌拉西林他唑巴坦治疗支气管扩张的临床观察

Clinical observation of alveolar wash by bronchoscope combined with piperacillin and tazobactam in the treatment of bronchiectasis

目的探讨支气管镜肺泡灌洗术联合哌拉西林他唑巴坦治疗支气管扩张的临床疗效。方法收集符合标准的患者80例,随机分为观察组和对照组各40例,两组患者均给予基础治疗,对照组加用哌拉西林他唑巴坦静脉滴注,观察组在此基础上合用支气管镜肺泡灌洗术,比较两组临床治疗效果。结果观察组患者临床症状缓解时间和住院时间均短于对照组,P<0.05。观察组临床治疗总有效97.50%,显著高于对照组的75.00%,P<0.05。治疗结束后,两组患者血气分析指标均上升,与治疗前比较均有显著性差异(P<0.05),但是观察组改变幅度更大(P<0.05)。结论支气管镜肺泡灌洗术联合哌拉西林他唑巴坦治疗支气管扩张,可以缩短患者临床症状改善时间和住院时间。

Objective To explore the clinical therapeutic effect and safety of alveolar wash by bronchoscope combined with piperacillin and tazobactam in the treatment of bronchiectasis.Methods 80 patients meeting the criteria were randomly divided into the observation group and the control group,40 patients each group.All patients were given the basic treatment and supplemented with intravenous dripping of piperacillin and tazobactam,and the observation group was treated with alveolar wash by bronchoscope additionally.The clinical therapeutic effect was compared between the two groups.Results The remission time of clinical symptoms and the duration of hospital stay were obviously shorter in the observation group than in the control group with statistical significance（P0.05）.The total effective rate of the observation group was 97.50%,which was significantly higher than that of the control group（75.00%）（P0.05）.The indexes of blood gas analysis both increased in the two groups after the treatment,but the increase of the observation group was higher than that of the controls（P0.05）.Conclusion Alveolar wash by bronchoscope combined with piperacillin and tazobactam can shorten the time of clinical symptom improvement and the duration of hospital stay in the treatment of patients with bronchiectasis.
