脑钠肽在慢性阻塞性肺疾病合并呼吸衰竭患者中的应用

Application of brain natriuretic peptide in monitoring chronic obstructive pulmonary disease with respiratory failure

脑钠肽(BNP)作为重要的心脏标志物在调节血管张力、体液平衡及心血管功能方面起重要作用。心室壁张力及压力负荷过重等均能刺激BNP的合成、分泌、释放,其水平能反映心室收缩和舒张功能的严重状态。慢性阻塞性肺疾病(COPD)合并呼吸衰竭患者常伴心功能不全或衰竭,BNP能用于COPD合并呼吸衰竭患者的病情严重程度评估、预后评价。本文就近年来BNP与COPD合并呼吸衰竭的相关研究进展做一综述。

As an important cardiac markers, brain natriuretic peptide （BNP） plays important roles in the regulation of systemic vascular resistance, fluid balance and cardiovascular function. Ventricular wall tension and pressure overload can stimulate the synthesis secretion and the release of BNP. The level of BNP is clinically useful for the di- agnosis of decompensatory heart failure and the prognosis of heart failure. Chronic obstructive pulmonary disease （COPD） patients with respiratory failure is associated with cardiovascular function impairment or failure. It is demon- strated that BNP plays an important role in the evaluation of the severity, as well as the treatment and prognosis of chronic obstructive pulmonary disease patients with respiratory failure.
