摘要
目的探讨磁共振成像(MRI)对甲状旁腺功能减退脑部异常钙化的诊断价值。方法收集15例甲状旁腺功能减退伴颅内异常钙化的MRI资料,结合解剖及病理分析影像表现。结果 15例颅内异常钙化呈对称性分布,钙化位于豆状核15例、尾状核及小脑齿状核及丘脑各14例、白质半卵圆中心10例、皮质下区6例、脑桥3例。钙化形状与钙化沉着解剖形态相似。核团钙化T1W均呈稍高信号,而T2W呈稍高信号9例、等信号3例、稍低信号3例。皮质下区钙化T1W呈稍高信号、T2W呈等信号,而白质半卵圆中心钙化T1W呈等信号、T2W呈稍高信号,两区域钙化呈分水岭样分布。结论 MRI能准确显示甲状旁腺功能减退所致脑部钙盐沉着的特征,特别是钙化信号的多样表现,一定程度上反映了钙盐沉着与解剖及病理有关。
Objective To investgate the value of magnetic resonance imaging (MRI) findings in the diagnosis of intracerebral calcification in hypoparathyroidism(HP). Methods Fifteen patients, which had intracerebral calcification because of HP, were analyzed MRI findings with anatomy and pathology. Results 15 patients had symmetrical distribution of intracerebral calcification. The calcification was located in lentiform nucleus (n= 15), caudate nucleus (n= 14) ,dentate body of cerebellum (n= 14) ,thatamus(n= 14) ,oval center of alba (n= 10), area under cortex (n= 6) , brain stem (n= 3). The shape of calcification was related to its anatomical site, The intracerebral nucleus' calcification displayed hyperintense on T1W, but displayed hyperintense (n=9) and isointense (n=3) and hypointense (n=3) on T2W. The calcification of area under cortex displayed hyperintense on TlW and isointense on T2W, the calcification of oval center of alba displayed isointense on T1W and hyperintense on T2W, the two areas' calcification had a watershed of distribution. Conclusion MRI can exactly display the character of intracerebral calcification in hypoparathyroidism, especially the different signals of calcification,by reflecting the feature of ealcific deposition being related to anatomy and pathology.
