山东省乡镇卫生院卫生人力资源及基本医疗服务现状调查

Status quo of health manpower and primary health service in township health clinics of Shandong province

目的:了解山东省乡镇卫生院卫生人力资源分布现状及基本医疗服务开展情况,为合理配置乡镇卫生院卫生人力资源及更好地开展基本医疗卫生服务提供数据支持。方法:以经济发展水平作为分层依据,采用分层整群抽样方法对山东省17个地市的80所乡镇卫生院进行现场调查。结果:山东省乡镇卫生院平均拥有卫生技术人员50.85人,高于2008年全国乡镇卫生院的平均水平。中心乡镇卫生院的平均床位数、下属卫生室/站个数和服务行政村个数多于一般乡镇卫生院;乡镇卫生院卫生技术人员以专科和中专为主,占81.24%,初级技术职称及无技术职称者分别占58.63%和8.48%,4.65%的专业技术人员不具备执业资格;开展家庭出诊、家庭护理、家庭病床的乡镇卫生院分别为42.50%、22.50%和25.00%,乡镇卫生院在家庭诊疗服务,社区院前急救服务,转诊服务,中医药服务方面开展比率偏低。结论:乡镇卫生院卫生技术人员配置数量尚可,但学历层次和专业技术职称偏低,基本医疗卫生服务内容有待进一步完善。

Objective To understand the status quo of health manpower distribution and primary health service conducted in township health clinics of Shandong province,and to provide statistic reference for rational allocation of health manpower in township health clinics and better primary health service.Methods We stratified 80 township health clinics of 17 cities in Shandong Province by the level of economic development,and made on-the-spot investigation by stratified clustering sampling.Results There was an average of 50.85 health technical staffs in each township health clinic,which was higher than the national level.There were more beds,subordinate health rooms and serving administrative villages in township health centers than township health clinics.Health technical staffs were mainly graduated from junior college or technical secondary school（81.24%）.The proportions of staffs with primary or without professional title were 58.63 and 8.48%,respectively.4.65% of the staffs hadn＇t acquired the qualifications of professional practice.The proportion of township health clinics conducting family health services was 42.50%,family nursing 22.50%,family bed service 25.00%.There were fewer township health clinics to provide household health service,community first aid service,transferral service and traditional Chinese medical service.Conclusion The allocation quantity of health technical staffs is rather satisfying,yet the professional title and educational background of which were still low.Primary health service in the township health clinics is to be improved.
