摘要
目的分析药师审核处方对合理用药的意义。方法对2011年1月至2012年11月急诊药房《用药错误干预记录》的错误处方进行分析统计。结果药师干预错误处方110份,主要错误为选药不合理、用药剂量错误、使用溶媒错误、配伍禁忌等。结论通过药师审核并干预错误处方,可有效减少和防范用药错误的发生,保障临床用药安全、有效、合理。
Objective To analyze the significance of pharmacists＇ examination and intervention of prescriptions for rational use of drugs. Methods The record of wrong prescriptions intervened from January 2011 to November 2012 in the Emergency Pharmacy were analyzed. Results A total of 110 prescriptions were intervened by pharmacists. Main error included irrational drug selection, and dosage errors, the use of solvents error, and incompatibility. Conclusion Examination and intervention of prescriptions by pharmacists can effectively reduce and prevent the occurrence of medication errors, ensuring the safety of clinical medication.
