老年急性胆囊炎的临床治疗体会

目的:探讨老年急性胆囊炎的临床特点、手术时机以及手术特点。方法:收治老年急性胆囊炎患者66例,回顾性分析临床资料。结果:66例患者中,行单纯性胆囊切除术57例,胆囊大部切除5例。胆囊切除+胆总管探查T管引流术3例,胆囊造瘘1例,治愈64例,死亡2例,术后并发症9例,肺部感染6例,切口裂开2例,切口疝1例。结论:老年急性胆囊炎早期明确诊断,无手术禁忌时应及早行手术治疗。加强围手术期的处理,是提高治愈率的关键。

Objective：To investigate the clini- cal characteristics of elderly patients with a- cute cholecystitis, operation time and opera- tion characteristics. Methods：a retrospective analysis of 66 cases of acute cholecystitis in senile patients a clinical data. Results： 66 patients in simple cholecystectomy in 57 ca- ses. subtotal cholecystectomy 5 cases. Chole- cystectomy ＋ common bile duct exploration and T tube drainage in 3 cases, gallbladder fistula in 1 cases,64 cases were cured,2 ca- ses died,9 cases of postoperative complica- tion,pulmonary infection in 6 cases,incision dehiscence 2 cases,incision hernia in 1 ca- ses. Conclusion： acute cholecystitis in the elderly early diagnosis, operation should be no taboos and early operation treatment. Strengthen surround operation period pro- cessing, is the key to increase the cure rate.
