胰管内乳头状黏液囊腺瘤诊断和治疗新进展 被引量:2

摘要 胰腺囊性肿瘤包括浆液性囊腺瘤、黏液性囊腺瘤(癌)、囊性内分泌肿瘤、囊性假乳头状瘤及胰管内乳头状黏液囊腺瘤(intraductal papillary mucinous neoplasms, IPMN)。WHO将IPMN定义为由乳头状增生的黏液分泌上皮细胞构成的、伴或不伴过度黏液产生及胰管囊性扩张的特殊病理类型,其比例占原发性胰腺囊性肿瘤的20%-25%。IPMN为癌前病变,与结直肠癌的发病过程类似,遵循腺瘤一腺癌的发展顺序,病变过程极其缓慢,通常约15-20年。日本学者于1982年首次报道了4例IPMN,并描述了3种特征性的影像学表现:主胰管扩张、壶腹部开口扩张、黏液溢出。近年来由于新的影像学、分子生物学和内镜技术的引入以及患者就诊意识的提高,IPMN的发现率较以往大大提高。近10年来对IPMN的诊断和治疗增加了较多新的认识。
