两种不同护理方法的动脉导管采血法对血常规结果的影响

Two different caring methods for arterial catheter blood affecting result of blood routine

目的研究两种不同方法护理的有创动脉导管采集的血液标本对血常规检测结果的影响。方法桡动脉血与肘静脉血血常规结果比较时,对同一患者同时在桡动脉与肘静脉抽血2 ml,注入乙二胺四乙酸二钾(EDTA-K2)抗凝的真空试管,充分摇匀。肘静脉血与肝素水冲管动脉血的血常规结果比较时,肝素水动脉采血时,用5 ml注射器取4 ml血液丢弃,再用另一支5 ml注射器取2 ml血液注入EDTA-K2抗凝的真空试管,充分摇匀。生理盐水冲管采血方法与肝素水冲管采血方法相同;肘静脉血作为对照组,所有标本30 min内送检,2 h内出结果,对其检测结果进行分析。结果桡动脉血组与肘静脉对照组的白细胞(WBC)、红细胞(RBC)、血红蛋白(HGB)、血小板(PLT)检测结果差异无统计学意义(P>0.05);肝素水试验组与肘静脉对照组WBC、RBC、HGB、PLT检测结果差异都有统计学意义(P<0.05),肝素水冲管动脉血的WBC、RBC、HGB、PLT检测结果比肘静脉对照组低;生理盐水试验组与肘静脉对照组WBC、RBC、HGB、PLT检测结果差异无统计学意义(P>0.05)。结论动静脉血均宜用于血常规检测,肝素水冲管动脉血不宜用于血常规检测,生理盐水持续加压滴入的有创桡动脉导管可以作为血常规标本的采血通道,结果准确,又可以减轻患者的痛苦。

Objective To study the impact of blood routine results of two different caring methods for invasive arterial catheter blood samples collected.Methods Collected 2 ml blood from radial artery and venous in the same patient and shook well when we compared radial artery and venous blood.Collected 4 ml blood with 5 ml syringe and took away when we compared to heparin flush pipe of arterial and venous blood.Collected 2 ml blood from radial artery and shook well.The way of blood collection of saline flush pipe was the same of heparin flush pipe.The control group was cubital venous blood.All specimens must be sent to clinical laboratroy in 30 minutes and tested in 2 hours and analyzed the results.Results The results of WBC,RBC,HGB,PLT showed no statistically difference between arterial and venous blood（P0.05）.There was statistically significant difference between heparin test group and cubital vein group（P0.05）.The results of WBC,RBC,HGB,PLT of heparin flush pipe of arterial blood were lower than those of venous blood.The results of WBC,RBC,HGB,PLT showed no statistically difference between saline test group and venous blood（P0.05）.Conclusions Both arterial and venous blood could be tested for blood routine and the arterial blood of herparin flush pipe should not be used for test of blood routine.The access of sustained pressure saline drip to invasive radial artery catheter could be used to collected blood for blood routine sample.It could reduce the suffering of patients and the results are accurate.
