医护患一体化模式在头颈肿瘤病房优质护理服务中的应用

Application of doctor-nurse-patient integration mode in quality nursing in department of head and neck cancer

目的探讨医护患一体化模式在头颈肿瘤病房优质护理服务中的应用效果。方法2011年7月至2012年6月构建并实施医护患一体化模式，进行医护联合查房，健康教育路径，流程再造，临床科研医护合作；2010年7月至2011年6月未实施医护患一体化模式。比较实施医护患一体化模式前后医生对护理工作了解率、护士对治疗方案了解率、护士对科研的兴趣和方法掌握情况、患者健康教育知晓率及满意度。结果实施前，医生对护理工作的了解率、护士对治疗方案的了解率、患者满意度分别为80．0％，85．0％，87．5％，实施后上述指标分别提高至100．0％，99．0％，95．8％，实施前后比较，差异均有统计学意义（X^2分别为8．720，15．231，4．364；P〈0．05）；实施后护士对科研有兴趣、文献查阅方法的掌握率和论文撰写方法的掌握率分别为100．0％，100．0％，75．0％，均高于实施前的35．0％，35．0％，35．0％，差异均有统计学意义（x^2分别为19．259，19．259，6．465；P〈0．05）。结论实施医护患一体化模式有利于医护沟通，提高患者满意度和健康教育的知晓率，促进护士对科研知识的掌握，培养科研兴趣，提升护理质量，深化优质护理服务内涵。

Objective To study the effect of doctor-nurse-patient integration mode in quality nursing in department of head and neck cancer. Methods Doctor-nurse-patient integration mode was established and implemented from July 2011 to June 2012, and doctor-nurse ward visits, health education pathway, process reengineering and health care cooperation in clinical research were conducted. Doctors＇ knowledge of nursing, nurses＇ knowledge of therapeutic schedule, nurses＇ interests in scientific research and grasp of methods, patients＇ knowledge of health education and satisfaction were compared before and after doctor-nurse-patient integration mode. Results Doctors＇ knowledge of nursing, nurses＇ knowledge of therapeutic schedule and patients＇ satisfaction were respectively 80.0% , 85.0% and 87.5 before the implementation of doctor-nurse- patient integration mode, and increased to 100.0%, 99.0% and 95.8% after, with statistically significant differences （ X^2 = 8. 720,15. 231,4. 364, respectively; P 〈 0.05 ）. Nurses＇ interests in scientific research, grasp of literature consulting methods and paper writing methods were respectively t00.0% , 100. 0% and 75.0% after doctor-nurse-patient integration mode, all of which were higher than before （35.0% , 35.0% and 35.0% ）, and the differences were statistically significant （X^2 = 19. 259,19. 259,6. 465, respectively; P 〈 0.05 ）. Conclusions Implementation of doctor-nurse-patient integration mode can promote doctor-nurse communication, improve patients＇ satisfaction and knowledge of health education, advance nurses＇ grasp of research knowledge, boost their interests in scientific research, enhance nursing quality and deepen the connotation of quality nursing.
