非酮症高血糖性舞蹈病16例临床及影像学分析 被引量:7

摘要 目的对16例非酮症高血糖性舞蹈病患者的临床表现、实验室检查和影像学资料进行分析,探讨其发病机制。方法回顾性分析16例非酮症高血糖性舞蹈病患者的临床资料。结果 16例患者的年龄为50~88(72.8±9.5)岁,女性占56.25%。6例(37.5%)为首次诊断糖尿病。入院急查血糖为5.61~38.90(19.60±8.49)mmol/L,血渗透压为288.84~322.26(309.95±8.47)mosm/L。16例均为偏身舞蹈,9例累及头面部,1例伴痫性发作。头CT多数(12/16)显示舞蹈肢体对侧或双侧基底节区稍高密度,头MRI(8/9)相应区域显示T1高信号,T2稍低(5/8)、等(2/8)或高信号(1/8)。壳核(8/8)均受累,部分尾状核(2/8)受累。上述影像学改变经过数月后逐渐减弱或消失。多数病例(15/16)经控制血糖和应用多巴胺受体拮抗剂病情得到改善。随访8例,3例因血糖控制不良(1例)或早期停用多巴胺受体拮抗剂(2例)而复发。结论非酮症高血糖性舞蹈病是一组可逆的临床综合症,纠正高血糖可以迅速改善病情。本病机制尚不明确,代谢异常和糖尿病中枢神经系统微血管病变可能起重要作用。
