3D-MR在特殊类型腰椎间盘手术中的应用 被引量:4

摘要 目的探讨3D-MR对手术治疗特殊类型(包括游离型及外侧型)腰椎间盘突出症的应用价值。方法运用3D-MR手术治疗特殊类型腰椎间盘突出症患者35例,根据患者的术前影像、突出部位、临床症状,以及3D-MR下神经、突出椎间盘的关系进行术前设计,制定手术方式,采用开窗、半椎板减压髓核摘除或微创椎间孔镜突出椎间盘切除术,或结合内固定融合进行手术治疗,应用VAS及JOA评分评价患者术后症状与神经功能恢复情况,分析手术过程中容易出现的问题及操作技巧。结果术后随访(12.39±6.4)个月,无失访。所有患者均未出现脑脊液漏、感染、神经功能减退等不良事件,术口均一期愈合。术后2周及末次随访时VAS评分均比术前减少,JOA评分均比术前增加,差异有统计学意义(P<0.05);末次随访时VAS评分比术后2周减少,JOA评分比术后2周增加,差异有统计学意义(P<0.05)。结论 3D-MR能够很好地阐明椎间盘突出与神经根、椎间盘的关系,进而在三维定位的基础上完成术前设计;三维定位毗邻关系有助于充分暴露、精细分离,减少损伤的手术步骤与技巧可以帮助术者进一步规范手术思路,突出的椎间盘的形状特点能够更好地指导术中是否存在残余;减少髓核的遗漏,获得良好的临床疗效。
