早期应用无创正压通气治疗COPD急性加重期患者的有效性与安全性研究 被引量:11

Efficacy and Safety of Early Noninvasive Positive Pressure Ventilation in Treatment of Patients with an Acute Exacerbation of Chronic Obstructive Pulmonary Disease

摘要 目的研究早期应用无创正压通气治疗慢性阻塞性肺疾病(COPD)急性加重期患者的有效性与安全性。方法将黔江中心医院2011年1月-2012年2月收治的72例COPD急性加重期患者随机分为两组,对照组35例采用常规治疗,观察组37例患者采用常规治疗+早期应用无创正压通气治疗,比较两组的疗效及安全性。结果观察组的总有效率为97.3%,明显高于对照组的74.3%,组间差异有统计学意义(P<0.05);两组治疗后的肺功能指标均明显改善,且观察组较对照组的FEV1和FVC改善更为明显(P<0.05);两组患者均无严重不良反应发生。结论早期应用无创正压通气治疗COPD急性加重期患者的有效性与安全性均更为满意,并且能够明显改善患者的肺功能,是临床首选的治疗方案。 Objective To study the efficacy and safety of early noriinvasive positive pressure ventilation (NPPV) in the treat- ment of patients with an acute exacerbation of chronic obstructive pulmonary disease (COPD). Methods From January 2011 to February 2012 in our hospital, 72 patients with an acute exacerbation of COPD were selected and randomly divided into two groups, the control group (35 cases) received conventional therapy, while the observation group( 37 cases) received early application of NPPV based on the conventional therapy. The efficacy and safety in both groups were compared and analyzed. Results The total effective rate of treatment group was 97.3% , significantly higher than 74.3% of the control group, the difference was statistically significant between two groups( P 〈 0.05 ) ; after the treatment, the pulmonary function indicators of both groups were improved significantly, and FEV1 and FVC of the observation group was improved more significantly ( P 〈 00. 05 ) ; There was no serious adverse reaction in both groups. Conclusion Early application of NPPV in the treatment of patients with an acute exacerbation of COPD has more satisfactory clinical efficacy and high safety, and can significantly improve patients' lung function. It is the preferred regime for COPD.
