摘要
目的探讨ki-67,MMP-9与子宫内膜腺癌(EC)临床病理特征的关系。方法采用免疫组化通用型二步法(PV-9000)检测15例正常增生期子宫内膜、20例不典型增生子宫内膜及56例子宫内膜样腺癌组织中ki-67及MMP-9蛋白表达情况,并观察两者表达相关性。结果子宫内膜腺癌组织与不典型增生子宫内膜、正常增生期子宫内膜组织相比,ki-67及MMP-9蛋白的阳性表达率均增加,差异有统计学意义(x^2=6.249,P=0.044;x^2=9.683,P=0.008),子宫内膜腺癌组织中ki-67和MMP-9的表达呈正相关(r=0.334),且它们的阳性表达率与EC的组织分化程度(ki-67:x^2=7.054,P=0.029;MMP-9:x^2=7.049,P=0.029)、临床分期(ki-67:x^2=17.422,P=0.000;MMP-9:x^2=18.971,P=0.000)及淋巴结转移(ki-67:x^2=13.440,P=0.000;MMP-9:x^2=8.429,P=0.004)密切相关。结论 ki-67,MMP-9与子宫内膜腺癌的增殖和侵袭有关,联合检测子宫内膜腺癌中ki-67与MMP-9的表达,有利于对子宫内膜腺癌的生物学行为和预后的判定。
Objective To investigate the characteristic expression of ki-67 and matrix metalloproteinase 9(MMP-9) in malignant endometrium. Methods In fifty-six patients with malignant endometrium,fifteen patients with proliferative phase of endometrium and twenty patients with atypical hyperplastic endonleirium,their expression of ki-67 and MMP-9 were studied by immunochemistry PV-9000 2-step method. Results The positive expression rate of ki-67 and MMP-9 in nmlignant endometriunl were hoth showed statistically higher than that in proliferative phase of endometrium and in atypical hyperplastie endometrium, and there were significant differences statistically (X2 = 6. 249, P = 0. 044 ;X2 = 9. 683 ,P = 0. 008). Ki-67 expression was positively correlated with MMP-9 expression in malignant endometrimn( r = 0. 334 ) , and the expression levels of Ki-67 and MMP-9 were closely correlated with the pathologic differentiation ( ki-67 :X2 = 7. 054,P = 0. 029 ; M MP-9 :X2= 7. 049, P = 0.029 ), clinic stages ( ki-67 :X2 = 17. 422, P = 0. 000 ; MMP-9 :X2 = 18.971, P = 0.000 ) and lymph metastasis ( ki-67 : X2 = 13. 440, P = 0. 000 ; MMP-9 :X2 = 8. 429 ,P = 0.004 ). Conclusion Ki-67 and MMP-9 expression have a close relationship with the cell proliferation and invasion of endometrial carcinoma. Combined detection of ki-67 and MMP-9 expression is helpful fi^r the evaluation of tumor biological behavior and prognosis in patients with malignant endometrium.
