医院不合理用药情况分析

Analysis of irrational drug use in hospital

目的调查我院门诊用药情况，对用药不合理现象进行分析，为促进我院临床合理用药提供参考。方法随机抽取我院2010年1月至2012年12月的门诊处方，每月随机抽取2d，并随机抽取该日的门诊处方100张，共计4800张处方，对这些处方的用药合理情况进行统计分析。结果4800张样本处方中，用药合理的处方有4337张，占总样本量的90．4％。有用药不合理现象的处方463张，占总样本量的9．6％。用药不合理处方中，具体用药不合理的现象及所占比例分别为：大处方、多药合用（33张，7．1％），药物用量不当（54张，11．7％），选用药物不当（68张，14．7％），中药注射剂的应用不当（86张，18．6％），药物配伍使用不当（95张，20．5％），以及抗菌药物应用不合理（127张，27．4％）。结论我院临床药物应用情况基本合理，但仍有部分医师、药师在药物选用方面存在纰漏。医院应加强管理，积极制定监督措施，促进医院科学合理地用药。

Objective To analyze the rationality of the clinical use of drugs by investigating in our hospital outpatient medication and analyze medication unreasonable phenomenon. Methods The outpatient prescriptions in our hospital during 2010 and 2012 were randomly selected. 100 outpatient prescriptions each day two days in each month were randomly selected, a total of 4800 prescriptions were used for statistical analysis. The reasonable medication use circumstances of these prescriptions were analyzed. Results Sample of 4800 cases prescription reasonable prescription medication 4337 were randomly selected, accounting for 90.4% of the total sample size. Prescrip tion medication unreasonable phenomenon 463 cases, accounting for 9.6% of the total sample size. Medication unreasonable prescriptions included inappropriate medication different groups（33,7.1% ）, drug dosage improper（54, 11.7% ）, inappropriate use drugs （68, 14. 7% ）, administration solvent inappropriate choice （86, 18.6% ）, the drugs combined use improper （ 95,20.5% ）, as well as antibacterial unreasonable application （ 127,27.4% ）. Conclusions The clinical usage of medicines in the hospital is rational as a whole. However there are still some physicians, pharmacists flaws exist in the area of drug selection. Hospitals should strengthen management, and actively develop oversight measures, to promote scientific and reasonable hospital medication.
