摘要
目的探讨在胃癌中IGF-Ⅰ、IGF-ⅠR及IGF-Ⅱ的表达以及与临床病理的关系。方法取原发胃癌组织、距胃癌组织5cm以上癌旁组织的标本共计40例和胃溃疡切除后经病理证实为正常胃组织的标本共计10例,采用免疫组织化学SP法检测3组IGF-Ⅰ、IGF-ⅠR、IGF-Ⅱ的表达,将结果与患者的临床病理指标进行综合分析。结果癌旁组织和正常胃组织中IGF-Ⅰ的表达率明显低于胃癌组织(P<0.05)。PTNM分期中Ⅰ~Ⅱ期低于Ⅲ~Ⅳ期(P<0.05);淋巴结阴性组低于淋巴结阳性组(P<0.05);未侵及全层组低于侵及全层组(P<0.05)。癌旁组织和正常胃组织中IGF-ⅠR的表达率明显低于胃癌组织(P<0.05)。PTNM分期中Ⅰ~Ⅱ期低于Ⅲ~Ⅳ期(P<0.05);淋巴结阴性组低于有淋巴结阳性组(P<0.05);未侵及全层组及侵低于全层组(P<0.05)。癌旁组织和正常胃组织中IGF-Ⅱ的表达率明显低于胃癌组织(P<0.05)。PTNM分期中Ⅰ~Ⅱ期低于Ⅲ~Ⅳ期(P<0.05);淋巴结阴性组低于有淋巴结阳性组(P<0.05);未侵及全层组低于侵及全层组(P<0.05)。结论 IGF-Ⅰ、IGF-ⅠR、IGF-Ⅱ的显著表达与胃癌的发生发展有关,且在胃癌的侵袭和转移中起重要作用。
Objective To investigate the correlation between of the expressions of insulin-like growth factor-Ⅰ( IGF-Ⅰ), insulin-like growth factor-Ⅰ receptor( IGF-Ⅰ R) and insulin-like growth factor-II (IGF-Ⅱ ) and clinical pathological characteristics including histological type, PTNM stage, infiltration as well as metastasis of gastric carcinoma ( GC ). Methods The expressions of IGF- Ⅰ , IGF- Ⅰ R, and IGF-Ⅱ were detected by immunohistochemistry(SP) in total 40 specimens including GC tissues, margin tissues which were more than 5em far from primary focus and 10 cases of normal gastric mucosa. The detection results were analyzed and compared with patients' clinical pathological parameters. Results The expression rates of IGF-Ⅰ in margin tissues and normal gastric mucosa were significantly lower than those in GC group ( P 〈 O. 05 ). The expression levels of IGF- Ⅰ were closely correlated to the tumor PTNM stage, lymph node metastasis and depth of infiltration ( P 〈 O. 05 ). The expression rates of IGF- I R in margin tissues group and normal gastric mucosa group were significantly lower than those in GC group ( P 〈 O. 05 ). The expression levels of IGF-ⅠR were closely correlated to tumor PTNM stage,lymph node metastasis and depth of infiltration ( P 〈 0.05 ). The expression rates of IGF-Ⅱ in margin tissues group and normal gastric mucosa group were significantly lower than those in C,C group( P 〈0.05). The expression levels of IGF-Ⅱ were closely correlated to tumor PTNM stage,lymph node metastasis and depth of infiltration ( P 〈 O. 05). Conclusion The overexpression of IGF-Ⅰ, IGF-ⅠR, and IGF-Ⅱ is closely correlated to carcinogenesis and development of gastric carcinoma,which may play an important role in the invasion, infiltration and metastasis of GC.
