摘要
目的通过动物实验研究反复快速扩张方式的扩张机制,为临床研究提供理论依据。方法 62只雄性新西兰大白兔随机分为反复快速扩张组(A组)、快速扩张组(B组)、常规扩张组(C组)、植入不扩张组(D组)和正常对照组(E组)。5组于不同时期取扩张皮肤标本进行HE染色及EGF免疫组织化学染色对比观察。结果组织学观察:A、C组在不同维持期组织学表现接近。B组在维持期1~3周均有大量胶原纤维断裂,且表皮层厚度增加不明显。D、E组无区别。EGF结果:在维持1周A、B、C组表达无差异无统计学意义(P>0.05),维持2周A、B差异无统计学意义(P>0.05),2组均与C组差异有统计学意义(P<0.05),维持4周3组表达差异无统计学意义(P>0.05)。结论反复快速皮肤扩张可能通过促进组织细胞合成与分泌EGF来提高扩张皮肤的修复质量;反复快速皮肤扩张方式可以有效缩短皮肤扩张修复治疗疗程。
Objective To provide theoretical basis for the clinical application of repeated rapid skin expansion by means of animal experimental study. Methods 62 male New Zealand white rabbits were randomly divided into three groups, repeated rapid expansion group (group A ), rapid expansion group (group B ), conventional expansion group ( group C ), implant group without expansion ( group D ) and normal control group (group E). The skin specimens at different phases were taken out to be fixed in 10% formaldehyde solution,then the sections were made and stained with HE and immunohistochemistry. The statistical analysis for the detection results were performed by statistical software SPSS 11. 5. Results Histological observation: the histological characteristics in group A were similar to those in group C at different maintain phases. In group B, a great deal of collagen fiber splitting was found during 1 ~3 weeks at maintain phases, however, the changes of epidermis layer thickness were not obvious, and there were no differences between group D and group E. The EGF detection results:there were no significant differences in the expression levels of ECG among group A, B, C, at maintain 1 week ( P 〉 0.05), there were no significant differences in the expression levels of ECG between group A and group B at maintain 2 weeks ( P 〉 0.05 ), however, there were significant differences in the expression levels of ECG between group A, B and group C ( P 〈 0.05 ), but there were no significant differences in the expression levels of ECG among group A,B,C at maintain 4 weeks( P 〉0.05). Conclusion The repeated rapid skin expansion may improve repairing quality of skin expansion by promoting the synthesis and secretion of ECG in histiocytes, furthermore, repeated rapid skin expansion can effectively shorten the treatment course of skin expansion repairing.
