熊去氧胆酸与地塞米松联合应用治疗妊娠期肝内胆汁淤积症的疗效分析

Clinical Effect Analysis of Combined Application of Ursodeoxycholic Acid and Dexamethasone in Treatment of Intrahepatic Cholestasis of Pregnancy

目的:探讨熊去氧胆酸与地塞米松联合应用治疗妊娠期肝内胆汁淤积症的临床疗效。方法:将我院2009年6月-2012年12月间收治的124例妊娠期肝内胆汁淤积症患者随机分为单纯地塞米松治疗组(对照组)和熊去氧胆酸联合地塞米松治疗组(观察组),各62例,比较两组患者的临床疗效。结果:两组患者治疗后ALT、AST和TBA水平较治疗前有显著改善,差异有统计学意义(P<0.05),观察组患者血清学检查指标优于对照组,组间差异有统计学意义(P<0.05)。观察组患者治疗后的瘙痒症状评分为(2.38±0.46)分,高于对照组的(2.10±0.41)分,组间差异有统计学意义(P<0.05)。结论:熊去氧胆酸与地塞米松联合应用治疗妊娠期肝内胆汁淤积症临床疗效好,安全性高,能够显著改善母婴结局,值得临床广泛推广使用。

Objective：To explore the clinical effect analysis of combined application of ursodeoxycholic acid and dexamethasone in treatment of intrahepatic cholestasis of pregnancy. Methods. 124 cases of intrahepatic cholestasis of pregnan- cy in our hospital from June 2009 to December 2012 were randomly divided into simple dexamethasone treatment group （control group） and ursodeoxycholic acid combined with dexamethasone treatment group （observation group）, 62 cases in each group, compared clinical curative effect of patients in two groups. Results.. After treatment, ALT, AST and TBA levels of patients were all improved significantly than before, the difference was statistically significant （P〈0. 05）, serology index of patients in observation group were better than that in control group, there was significant difference between two groups （P〈0. 05）. Pruritus score of patients in the observation group of was （2. 38 ±0. 46）, higher than that in the control group patients （2. 10±0. 41）, there was significant difference between the two groups （P〈0. 05）. Conclusion： Taking ursodeoxycholic acid combined with dexamethasone in treatment of intrahepatic cholestasis of pregnancy has good clinical curative effect, and has high safety, can improve maternal and child outcome significantly, it is worthy of extensive promotion.
