对比分析加巴喷丁与盐酸度洛西汀对糖尿病周围神经痛的临床疗效

Comparative analysis of clinical efficacy of gabapentin and duloxetine on diabetic peripheral neuropathic pain

目的对比分析加巴喷丁与盐酸度洛西汀对糖尿病周围神经痛的临床治疗效果与不良反应。方法将菏泽医学专科学校附属医院2010年2月至2011年8月收治的65例2型糖尿病伴糖尿病周围神经痛患者随机分成两组，观察组给予盐酸度洛西汀治疗，对照组给予加巴喷丁治疗，采用视觉模拟评分法（VAS）对疼痛情况做出评估。结果与给药前的VAS比较，观察组与对照组患者给药后第7、14、28天时的VAS均出现明显降低（P〈0．01），两组患者同期的VAS分值比较差异无统计学意义（P〉0．05），给药后两组同期有效率相比差异无统计学意义（P〉0．05）。观察组不良反应明显低于对照组，差异有统计学意义（P〈0．01）。结论采用加巴喷丁与盐酸度洛西汀治疗糖尿病周围神经痛均具有理想的治疗效果，但盐酸度洛西汀的不良反应发生率显著低于加巴喷丁，患者的依从性较好，值得临床推广应用。

Objective To compare the clinical effect and side effects of gabapentin and duloxetine on diabetic peripheral neuropathic pain（DPNP）. Methods Sixty-five patients with type 2 diabetes combined with DPNP admitted to the affiliated hospital of Heze medical college from February 2010 to August 2011 were randomly divided into two groups, the observation group were treated with duloxetine, and the control group received gabapentin, the pain were evaluated by visual analog scale （VAS）. Resuits Compared with the VAS before treatment, the VAS scores after treatment on the 7 th, 14 th and 28 th day of the two groups were significantly lower （ P 〈 0. 01 ）, there was no significant difference of the VAS scores between the two groups over the same period （P 〉 0.05 ）, and there was no significant difference between the two groups after administration of the same period in efficiency （P 〉 0.05 ）. The side effects of the observation group were significantly lower than that of the control group （P 〈 0. 01 ）. Conclusions Gabapentin and duloxetine treatment of diabetic peripheral neuropathic pain had the desired therapeutic effect, but the side effects of duloxetine were significantly lower than that of gabapentin, and the patients＇compliance is good and worthy of clinical application.
