适度调控中心静脉压对改善肺源性心脏病患者预后的临床观察 被引量:1

摘要 目的探讨适度调控中心静脉压对肺源性心脏病(简称肺心病)患者预后的影响。方法随机将120例失代偿期的肺心病患者分为对照组和治疗组,每组60例。对照组采用常规治疗,中心静脉压波动在10—15cm H2O;治疗组在常规治疗基础上应用扩血管、利尿等措施,调控中心静脉搏动在2~6cm H2O,观察两组治疗前后心功能改善情况及使用呼吸机时间。结果治疗组心功能改善优于对照组,使用呼吸机辅助通气时间明显短于对照组(P〈0.05)。结论调控中心静脉压可明显改善肺心病患者的预后。 Objective To investigate the impact of proper regulation of the central venous pressure on the prognosis of patients with pulmonary heart diseases. Methods One hundred and twenty cases of deeompensate pulmonary heart diseases were randomly divided into two groups, with 60 cases in each group. The check group was treated with routine method; the central venous pressures fluctuated between 10 and 15 em H2O. Based on the routine treatment, the treatment group was treated with methods such as vasodilatation, dieresis and etc. The central venous pressures were regulated to fluctuate between 2 and 6 cm H2O. The improvement situations of cardiac function of two groups before and after treatment and the time for patients of two groups to use ventilators were observed. Results The improvement of cardiac function of the treatment group was better than that of the control group, the auxiliary ventilation time for the treatment group to use ventilators was significantly less than that of the control group (P 〈 0.05 ). Conclusions Regulation of central venous pressure can significantly improve the prognosis of patients with pulmonary heart disease.
