微创关节镜下手术治疗盘状半月板破裂的疗效分析

Efficacy of arthroscopic surgery for treatment of discoid meniscus injury

目的探讨关节镜下手术治疗盘状半月板破裂的疗效分析。方法选取该科自2010年10月—2012年7月术中证实为盘状半月板破裂的患者共50例,其中部分切除成形术32例,次全切除术15例,全切术3例,采用刨削及低温射频消融切除的方法进行关节镜手术治疗,术前术后均采用Lysholm膝关节评分标准评判患肢膝关节的功能情况。结果术后所有患者均获得随访,平均7个月(3～10个月),复查患肢膝关节出现外侧关节间隙压痛阳性1例,负重行走疼痛3例,关节不稳2例,无伸直及屈曲受限,无弹响及交索。患肢膝关节Lysholm评分:50例盘状半月板损伤患者术前为(65.76±3.15)分,术后3个月为(89.64±4.03)分,手术前后比较差异有统计学意义(P<0.05)。结论关节镜手术治疗盘状半月板损伤,创伤小、术后关节功能恢复快、并发症少,能够最大程度上保留半月板。

Objective To analyze the method and curative effect of operation under arthroscopy for treatment of discoid meniscus injury. Methods 50 patients diagnosed discoid meniscus injury were included in this study from October 2010 to July 2012. Among them,par- tia,subtotal and total meniscectomy were performed by arthroscopy in 32,15 and 3 cases respectively. We used cuting and ablation with arthroscopy. We adopted Lysholm Score to assess the preoperative and postoperative knee joint function. Results All the cases had been followed up after operation —— an average of 7 months （3 - 10 months）. The review found that McMurray Sign still positive in 1 case, walk with pain 3 cases,instability of knee joint 2 cases without noise or lock in the joint. As for the Lysholm Scores of knee function, pre- operative Lysholm Scores of 50 cases of discoid meniscus injury were 65.76±4.19, and postoperative Lysholm Scores were 89.63 ±3.84. They had statistically significant difference between preoperation and postoperation. Conclusion It is demonstrated that arthro- seopic treatment for the discoid meniscus injury has less trauma, quick recovery of postoperative function of joint and few postoperative complications. It could reserve the meniscus to the greatest extent.
