磁共振扩散加权成像联合LAVA技术在肝脏占位病变中的临床应用研究 被引量:7

摘要 目的探讨磁共振扩散加权成像(DWI)联合LAVA技术在肝脏占位病变中的临床应用。方法随机选取2009年11月-2011年12月因各种肝脏占位病变就诊于我院的患者150例,回顾性分析患者的核磁共振检查的临床资料,比较A组(LAVA)、B组(DWI)以及C组(LAVA、DWI联用)在肝脏占位性病变的检出率、定性率及定性准确率。同时比较肝血管瘤、肝囊肿、肝癌的表观弥散系数(ADC)值,然后进行统计学分析。结果c组能够将病变定性的比例为64.5%、定性准确率为95.9%,均明显高于其他两组;肝囊肿在b=800s/mm^2时的ADC平均值为3.315±0.298,明显高于其他病变,其P均〈0.05。结论DWI联合LAVA技术能够显著提高肝脏占位性病变定性诊断的准确性,有很高的临床指导价值。 Objective To discuss the clinical effects of the combination of magnetic resonance diffusion- weighted imaging (DWI) and LAVA technology in diagnosis of hepatic space-occupying lesions. Methods One hundred and fifty cases are selected randomly from patients who were treated for various hepatic space-occupying lesions in our hospital from November 2009 to December 2011 and are divided into three groups: WI(Group A), LAVA technology(Group B) and combined DWI with LAVA technology(Group C). Through a retrospective analysis of the 150 patients' clinical information of nuclear magnetic resonance examination, comparisons of hepatic space-occupying lesions between three groups are made in the aspects of detectable rate, diagnosis rate and diagnosis accuracy rate. At the meantime, another comparison of hepatic hemangioma, hepatic cyst and liver cancer are necessary to be drawn between three groups in apparent diffusion coefficient(ADC) value. Then, the results of two comparisons stated mentioned hereinbefore need to be analyzed using statistic methods. Iqesults The diagnosis rate and the diagnosis accuracy rate of hepatic space-occupying lesions in Group C are 64.5% and 95.9% respectively, from which we can see that the clinical effect of Group C is obviously better than that of other two groups. In addition, the average ADC value of hepatic cyst in Group C reaches to 3.315i0.298 when b is equal to 800s/mm2, which shows an apparently higher ADC value of hepatic cyst than that of other diseases. All the results stated hereinbefore are obtained under the condition that P is less than 0.05. Conclusion DWI combined with LAVA technology can improve the accuracy rate for diagnosis of hepatic space-occupying lesions, which is of great significance in,clinical applications.
