阻塞性睡眠呼吸暂停低通气综合征患者上气道CT测量及临床意义 被引量:17

摘要 目的采用多层螺旋CT行上气道动态扫描,结合多平面后处理技术测量观察阻塞性睡眠呼吸暂停低通气综合征(OSAHS)患者上气道测量,判断阻塞平面,探讨其临床应用价值。方法选取经多导睡眠监测确诊的重度OSAHS患者20例和均无睡眠疾病方面主诉的健康志愿者(对照组)10名。对20例重度OSAHS患者及10名健康志愿者分别在平静呼吸和闭口堵鼻(Müller)吸气状态下行上气道CT扫描,测量各咽部截面积进行比较,判断阻塞平面。结果 CT显示重度OSAHS患者上气道截面积明显小于对照组,腭咽部和舌咽部截面积减小程度显著。结论采用螺旋CT对重度阻塞OSAHS患者行上气道动态扫描并进行截面积测量可有效判断其上气道阻塞情况,并对临床治疗方案选择提供帮助。 Objective To investigate the obstructed site in OSAHS patients using multi slices spiral computed tomography, and to estimate the value of CT in the diagnosis and treatment of OSAHS. Methods Twenty severe OSAHS sufferer who were viewed by PSG and ten normal adults who had no chief complaint of sleeping disease were collected. The curves of subjects in a respiratory cycle were recorded by respiratory monitoring system in PSG. The 20 patients with OSAHS and 10 controls underwent CT scan in the phases of quiet stable respiration and Muller action. The obstructed sites in upper airway were studied. Correlation analysis was carried out between the CT and clinical data. Results The cross section areas of upper airway in severe OSAHS were smaller than those of the controls, especial at the soft palate and tongue levels. The cross sections areas at the soft palate level under the quiet stable respiration and Muller action states had significant statistical difference between OSAHS and controls. Conclusion CT scan is helpful for assessing obstructed site of upper airway and determining therapy plan.
