30例全手皮肤套状撕脱伤临床疗效分析

Clinical outcomes analysis of 30 cases of degloving injuries of the hand

目的比较全手套脱伤三种不同治疗方法的临床疗效。方法随访我院2002年7月至2012年1月治疗的30例全手皮肤套脱伤患者。根据治疗方法不同，分为再植组（15例）、带蒂皮瓣组（5例）和游离组织组（10例）。按照手功能评定标准对三组患者的手功能情况进行评定。结果全手总失能百分比分别为再植组35．79％、带蒂皮瓣组82．80％、游离组织组68．15％。结论三种治疗全手套脱伤的方法，疗效从优到劣依次为再植、游离组织修复和带蒂皮瓣修复术。

Objective To compare the clinical outcomes of three different treatment methods for degloving injuries of the hand. Methods Thirty cases of degloving injury of the hand treated in our hospital from July 2002 to January 2012 were followed up. According to the different treatment methods, these cases were divided into replantation group （15 cases）, pedicled flap group （5 cases） and free tissue transplantation group （10 cases）. The hand function in the three groups was assessed according to the hand function evaluation criteria. Results The percentages of hand total disability were 35.79% in the replantation group, 82.80% in the pedicled flap group and 68.15% in the free tissue transplantation group. Conclusion The three treatment options for degloving injury of the hand, in the order from best to worst clinical outcomes, are replantation, free tissue transplantation and pedicled flap repair.
