全椎板切除和半椎板切除治疗老年腰椎管狭窄症临床效果对比观察

Comparison and Observation of Clinical Effect of Laminectomy and Semi-laminectomy in the Treatment of Elderly Lumbar Spinal Stenosis

目的对比观察全椎板切除和半椎板切除治疗老年腰椎管狭窄症的临床效果。方法对从2009年4月至2012年6月在我院手术治疗的52例老年腰椎管狭窄症患者进行统计分析,其中22例接受全椎板切除手术,30采用半椎板切除手术。对这52例患者术后随访7个月～3年,平均两年零6个月,采用JOA下腰痛手术疗效评分标准来评价手术效果。结果两种不同手术治疗后优良率分别为90.9%、93.3%,比较无明显差异。结论对老年腰椎管狭窄症患者,根据不同的病症采用不同的手术方法,都能获得满意的治疗效果。

Objective To compare and observe the clinical effect of laminectomy and semi-laminectomy in the treatment of elderly lumbar spinal stenosis. Methods 52 cases of elderly lumbar spinal stenosis patients conducted surgical treatment in our hospital from April 2009 to June 2012 were selected as the research object and statistically analyzed. Among 52 patients, 22 cases were given laminectomy surgery, and 30 cases were given semi-laminectomy surgery. All patients were followed up for 7 months to 3 years after the operation, with the average time of two years and six months. The operative effects of two methods were observed. Results The excellent and good rate of semi-laminectomy was 93.3%, and the excellent and good rate of laminectomy was 90.9%; the difference between two surgical methods was not statistically significant （P 〉0.05）. Conclusions For elderly patients with lumbar spinal stenosis, different surgical methods depending on the disease, can obtain a satisfactory therapeutic effect.
