调强放疗联合鸦胆子油乳注射液治疗早期宫颈癌28例临床观察

Treatment of early cervical cancer by intensity modulated radiation therapy combined with Brucea javanica oil emulsion injection

目的观察调强放疗联合鸦胆子油乳注射液治疗早期宫颈癌的临床疗效。方法将60例早期宫颈癌患者随机分为2组,治疗组28例予调强放疗联合鸦胆子油乳注射液治疗,对照组32例单纯应用调强放疗治疗。观察2组近期疗效,并研究治疗后1、2、3年病变局部控制率及治疗后1、2、3年总生存率,评价不良反应放射性直肠炎、放射性膀胱炎及骨髓抑制的发生率。结果 2组近期有效率和治疗后1、2、3年病变局部控制率比较差异均无统计学意义(P>0.05);治疗组治疗后3年总生存率高于对照组同期(P﹤0.05);2组放射性直肠炎Ⅰ～Ⅲ度发生率比较差异有统计学意义(P<0.05),2组放射性膀胱炎、骨髓抑制发生率比较差异无统计学意义(P>0.05)。结论调强放疗联合鸦胆子油乳注射液治疗早期宫颈癌,能够提高疗效和减少不良反应,值得临床推广应用。

Objective To observe the therapeutic effect of early cervical cancer by intensity modulated radiation therapy combined with Brucea javanica oil emulsion injection. Methods 60 patients with early cervical cancer were randomly divided into two groups. 28 cases in treatment group were treated by intensity modulated radiation therapy combined with Brucea javanica oil emulsion injection. 32 cases in control group were treated only by intensity modulated radiation therapy. The short -term effect,lesion local control rate and overall survival 1,2 and 3 years after treatment were observed. The incidence of radiation proctitis, radiation cystitis and bone marrow suppression were evaluated. Results There was no significant difference between two groups on the short - term effect and lesion local control rate （ P 〉 O. 05 ）. The 3 - year overall survival rate in the treatment group was higher than that in control group （ P 〈 O. 05）. There was significant difference between two groups on radiation proctitis （P 〈 0.05）, there was no sig- nificant difference between two groups on radiation cystitis and bone marrow suppression （ P 〉 0.05 ）. Conclusion Intensity modulated radiation therapy combined with Brucea javanica oil emulsion injection can improve the efficacy and reduce adverse reactions on early cervical cancer,worthy of clinical application
