P波离散度和QT离散度与慢性心力衰竭的相关性研究 被引量:3

摘要 目的探讨P波离散度(Pd)和QT离散度(QTd)与心力衰竭心肌肥厚的关系。方法选择慢性心力衰竭(CHF)患者60例作为观察对象(CHF组),心功能正常者50例作为对照组;两组均采用心电图测量Pd及QTd,心脏彩超测量舒张末期室间隔厚度(IVSTd)、舒张末期左室后壁厚度(LVP-WTd)、舒张末期左室内径(LVEDd)、左心房内径(LAd)等,并计算左室质量(LVM)。结果与对照组比较,CHF组患者LAd、IVSTd、LVP-WTd、LVEDd、LVM等显著增加(P<0.05);CHF组Pd和QTd与LVM呈正相关(r=0.446、0.387,P<0.05)。结论 Pd及QTd可作为一种无创性预测心力衰竭患者心肌重塑的手段。 Objective To investigate the relationship between P-wave dispersion(Pd),Q-T duration dispersion(QTd) with myocardial hypertrophy in cardiac failure. Methods 60 cases of chronic heart failure (CHF) were selected as the observa- tion subjects(CHF group) and the 50 cases of normal heart function as the control group. Pd and QTd were detected by electro- cardiogram, and the interventricular septum thickness at end-diastole (IVSTd), left ventricular posterior walldepth (LVP-WTd), left ventricular end-diastolic dimension (LVEDd),left atrial diameter (LAd) were determined by the cardiac color ultrasound. The left ventricular mass (LVM) was calculated. Results Compared with the control group, LAd, IVSTd, LVP-WTd and LVM in the CHF group were significantly increased (P〈0.05) ;Pd and QTd were positively correlated with LVM. Conclusion Pd and QTd can be used as the noninvasjve measure for predicting myocardial remodeling in the patients with heart failure.
