腹腔镜下卵巢囊肿剥除不同止血方法的观察

Observation on different hemostatic methods during laparoscopic ovarian cyst divesting surgery

目的探讨腹腔镜卵巢囊肿剥除术中使用不同止血方法对卵巢功能的影响。方法选择48例单侧卵巢良性囊肿患者进行腹腔镜下卵巢囊肿剥除术,按止血方式的不同分为两组:套圈结扎配合肾上腺素组为观察组,双极电凝为对照组,每组各24例。于术前、术后1个月和术后6个月检测E2、FSH、LH并随访。结果两组术后1个月时卵巢储备功能各项指标差异无显著性(P>0.05),术后6个月时对照组出现E2下降,FSH、LH水平显著高于观察组,差异有显著性(P<0.05)。结论腹腔镜卵巢囊肿剥除创面采用双极电凝止血会对卵巢功能造成损害,而使用套圈结扎配合肾上腺素止血对卵巢功能影响较小。

Objective Discuss the influence of ovarian function by using different method to hemostatic in the operation of laparoscopic ovarian cyst sipping.Methods Choose 48 cases of unilateral ovarian benign cyst patients for the operation of laparoscopic ovarian cyst sipping,Divided into two groups in line with the different way of bleeding： ring ligation with epinephrine group as observe group,and double arc setting as matched group,respectively 24 cases.In the preoperative and postoperative 1 month and after 6 month testing E2,FSH,LH and follow-up.Results Each index difference of ovarian reserve function assumes no statistical significance after 1 month（P 0.05）.Matched group performance for the drop of E2,FSH and LH level higher than the observation group significantly after 6 month,the difference assumes statistically significant（P〈0.05）.Conclusion Using bipolar an electrocautery unit blood will cause damage to ovarian function when sipping wound of laparoscopic ovarian cyst,however,using ring ligation with epinephrine hemostatic on ovarian function is less affected.
