不同时期骨质疏松性椎体压缩性骨折行经皮椎体后凸成形术的疗效分析 被引量:2

摘要 目的观察经皮椎体后凸成形术对不同时期骨质疏松性椎体压缩性骨折的疗效。方法回顾性分析2005年10月至2011年12月经皮椎体后凸成形术治疗94例老年单节段骨质疏松性椎体压缩性骨折患者的临床资料。根据术前骨折椎体内MRI信号强度不同分为新鲜期骨折组(A组,48例)、修复期骨折组(B组,30例)、基本愈合期骨折组(c组,16例)。比较三组治疗前后疼痛视觉模拟量表(VAS)评分、0swestry功能障碍指数(0DI)、椎体高度恢复、Cobb角矫正程度及骨水泥渗漏,评价治疗效果。结果94例患者均获得随访,随访12~25个月,平均14.3个月。术后第3天及末次随访时三组患者VAS评分、0DI较术前明显改善,差异有统计学意义(P〈0.05)。A组术后第3天及末次随访时VAS评分、0DI[(2.31±0.55)、(2.43±0.87)分,(20.89±4.57)%、(21.21±3.98)%]低于B组[(2.84±0.67)、(3.00±1.07)分,(22.55±5.01)%、(23.32±3.47)%]、C组[(2.90±1.07)、(3.45±1.54)分,(22.32±3.47)%、(25.97±3.94)%],B组低于C组,差异有统计学意义(P〈0.05)。三组术后第3天及末次随访时椎体前缘、中线高度与术前比较差异均有统计学意义(P〈0.05),末次随访时A组椎体前缘高度恢复率、椎体中线高度恢复率大于B、C组,B组大于C组,差异有统计学意义(P〈0.05)。三组术后第3天及末次随访时Cobb角与术前比较差异均有统计学意义(P〈0.05),末次随访时A组Cobb角矫正率大于B、C组,B组大于C组,差异有统计学意义(P〈0.05)。术中无脊髓、神经损伤以及球囊破裂。6例患者发生骨水泥渗漏,总渗漏率为6.38%(6/94),患者无临床不适。结论经皮椎体后凸成形术对不同时期骨质疏松性椎体压缩性骨折均能起到良好的止痛效果,对新鲜期的骨折在椎体高度及后凸角度恢复方面治疗效果更好,配合有效抗骨质疏松治疗可明显改善患者的生活质量。
