综合疗法治疗脑卒中后吞咽障碍诊疗技术推广

Promotion of Diagnostic and Therapeutic Technology of Post-Stroke Dysphagia Treated with Comprehensive Therapy

目的综合疗法脑卒中后吞咽障碍诊疗技术推广效果临床验证,推广模式可行性。方法以长春中医药大学附属医院脑病科为中心,以教、调、研相结合金字塔式辐射形式向吉林省多地区推广,从吉林省十个地区收集符合入选标准的625例患者,均采用针刺联合康复疗法综合治疗。结果治疗前后各中心患者洼田氏能力评定比较差异无统计学意义(P>0.05)。两个中心自身治疗前后吞咽困难评价标准量表比较差异均有统计学意义,说明两组均可以提高患者吞咽功能能力;总中心与分中心疗效基本一致,推广组与以往研究组疗效基本一致,达到推广效果。结论综合疗法治疗卒中后吞咽障碍能明显提高吞咽功能、提高生活质量、推广的方法及模式的有效性。

Objective To verified the effect of diagnostic and therapeutic technology promotion and the feasibility of the promotion mode in the treatment of post - stroke dysphagia with comprehensive therapy. Methods On the basis of the Encephalopathy Department of Affiliated Hospital of Changchun University of Traditional Chinese Medicine, the combined pattern of teaching, investigation and research was adopted for the pyramidal and radiating promotion in the regions of Jilin province. Six hundred and twenty - five patients who were in compliance with the inclusive standards were collected from ten regions in Jilin province. The comprehensive therapy of acupuncture and rehabilitation was applied. Results The assessment of swallowing function test did not present the significant difference before treatment for the patients in each center,indica- ting the comparability. In the self - comparison before and after treatment in two centers, the extremely signifi- cant difference presented in dysphagia assessment scale, indicating that the treatment in the two groups may all improve the swallowing function of patients. The efficacy of the headquarter center was basically same as the branch centers. The efficacy in the promotion group was basically same as the research group. It showed that the technology promotion had been targeted. Conclusion The comprehensive therapy apparently im- proves the swallowing function and life quality of the patients with post - stroke dysphagia. The promotion method and mode are effective.
