摘要
目的:了解部芩片治疗耐多药肺结核患者的临床疗效及对CD4+T、CD8+T细胞等淋巴细胞免疫功能的影响。方法:将52例耐多药肺结核患者随机分为(部芩片)治疗组、微卡组、对照组,3组分别给予西药加部芩片、西药加微卡注射液及单独西药治疗,疗程6个月,观察各组治疗前后临床疗效,检测其治疗前、治疗后2月、治疗后6月血CD4+T细胞和CD8+T细胞含量,比较CD4+/CD8+比值变化。结果:(1)与对照组比较,治疗后2月、6月微卡组和治疗组临床治疗有效率均增高,尤其是治疗组,治疗2月后有效率明显增高,差异有统计学意义。(2)与对照组比较,治疗2月后及6月后治疗组、微卡组CD4+T细胞含量、CD4+/CD8+比值均明显增高,差异有显著统计学意义,但各组CD8+T细胞含量无明显变化。(3)与治疗前比较,治疗2月后、6月后,治疗组及微卡组CD4+T细胞含量、CD4+/CD8+比值均明显增高,差异有显著统计学意义,而对照组治疗2月变化不明显,治疗6月后CD4+T细胞含量、CD4+/CD8+比值均明显增高,差异有显著统计学意义;治疗前后各组CD8+T细胞含量无明显变化。结论:部芩片通过改善患者咳嗽、咳痰等症状,提高治疗的痰菌阴转率,增高耐多药肺结核患者CD4+T细胞含量及CD4+/CD8+比值,提高机体细胞免疫功能,达到治疗耐多药肺结核的作用,其疗效和微卡相近。
To study the therapeutic effect and immunity reactions, such as CD4 + T and CDs + T of the Bu-qing tablet on MDR- TB patients. Methods :52 patients with MDR -TB were selected and randomly divided into Buqing tablet group, Vaccae group and control group, each group were given anti - tuberculosis drugs plus Buqing tablet, anti - tuberculosis drugs plus Vaccae(Freeze -dried M. vaccae for therapy), anti -tuberculosis drugs respectively. The clinical efficacy of three groups were observed, and the contents of CD4 + ,CD4 + and CD4 +/CD4 + were measured in the plasma pretherapy,2 months and 6 months after treatment, then these values were compared. Results:( 1 )The clinical efficacy of Buqing tablet group and Vaccae group were superior to that of control group, especially 2 months after treatment, that in Buqing tablet group was higher than that in control group in statistics. (2)2 months and 6 months after treatment,in statis- tics,the values of CD4 + and CD4 +/ CD4 + were higher in Buqing tablet group and Vaccae group than that in control group, but CDs + had no statistical difference. (3)In Buqing tablet group and Vaccae group, the values of CD4 + and CD4+ / CD4 + were higher 2 months and 6 months after treatment than that pretherapy, but CD4 + T had no statistical difference. In control group, the values of CD4 and CD4 +/CD8+ were higher 6 months after treatment than that preLherapy, but there was no statistical difference 2 months after treatment. CD4 + had no statistical difference during the treatment. Conclusions : By eliminating the symptoms, such as tussis and expectoration, improving the sputum negative conversion rate, Buqing tablet can treat MDR - TB patients by increasing the concentration of CD4 + and the ratios of CD4 + / CD4 + , and there is no difference between Buqing tablet and Vaccae in curative effect.
