控制性降压在鼻内窥镜术中的应用效果观察

Observation of the Effect of Controlled Hypotension Technique in Endoscopic Sinus Surgery

目的探讨硝酸甘油复合艾司洛尔控制性降压术在鼻内窥镜手术中的临床疗效。方法选择100例拟行鼻内窥镜手术的患者,ASAⅠ～Ⅱ级,随机分为两组,每组各50例,对照组行单纯硝酸甘油控制性降压,研究组采用硝酸甘油复合艾司洛尔控制性降压,术中检测各组出血量、手术时间、平均动脉压(MAP)以及心率(HP)。结果研究组出血量和手术时间均少于对照组,差异有统计学意义(p<0.05);研究组麻醉效果显著优于对照组,能更有效地控制脉压、心率,差异有统计学意义(p<0.05)。结论硝酸甘油复合艾司洛尔控制性降压术可显著减少出血量,缩短手术时间,防止发射性心率增加,是鼻内窥镜术较理想的麻醉方法。

Objective to explore the clinical efficacy of nitroglycerin combined with esmolol controlled hypotension in endoscopic sinus surgery.Method Selected 100 cases intended line of endoscopic sinus surgery patients,ASA I to II class,were randomly divided into 2 groups,each group of 50 patients in the control group with simple nitroglycerin controlled hypotension group nitroglycerin compound esmolol LosSeoul controlled hypotension,intraoperative detection of the bleeding,operative time,mean arterial pressure（MAP）,heart rate（HP）.Result Blood loss and operation time of the study group than the control group,the difference was statistically significant（p0.05）;the anesthetic effect of the study group was significantly better than the control group to more effectively control the pulse pressure,heart rate,the difference was statistically significant（p0.05）.Conclusion Nitroglycerin combined with esmolol in controlled hypotension technique could significantly reduce the amount of bleeding,shorter operative time,and prevent the increase of emission rhythm.It was one of the ideal method of anesthesia for endoscopic sinus surgery.
