血糖对心脏骤停患者预后的预测价值

摘要 目的观察在心脏骤停(CA)患者实施心肺复苏(CPR)后,血糖对预后的预测价值,为提高复苏成功率提供依据。方法回顾性分析CA患者120例,分为存活组(37例)和死亡组(83例),分别检测两组复苏过程中的血糖值,比较两组血糖水平的变化。结果死亡组开始复苏前血糖水平及复苏后各时点血糖水平均明显高于存活组(P<0.01);经过24 h复苏血糖水平高于10.0 mmol/L预测死亡,阳性率和阴性率达到100%。结论监测心脏猝死患者血糖水平,对预测患者的预后有一定的参考价值。 Objective To observe the predictive value of blood glucose in the prognosis of patients with cardiac arrest (CA) after they received cardiopulmonary resuscitation(CPR) in order to improve the successful rate of resuscitation. Methods Retrospective analysis was made in 120 patients with CA who were divided into survival group( n = 37 )and death group( n = 83 ). The blood glucose levels of both groups during the resuscitation were detected and the changes were compared between each other. Results The blood glucose levels before the resuscitation and at each time point after the resuscitation in the death group were significantly higher than those in the survival group( P 〈 0.01 ). After the resuscitation for 24 h, the level of 10.0 mmol/L blood glucose could be used as a reference critical value to predict death. The positive and negative rate all reached to 100%. Conclusion Monitoring blood glucose level of patients with sudden heart failure has certain reference value in the prediction of patients'prognosis.
