f摘要1目的探讨无创通气技术在辅助治疗肺源性心脏病(肺心病)合并Ⅱ型呼吸衰竭中的临床效果。方法将2010年7月~2012年10月在本院住院治疗的58例慢性肺源性心脏病合并Ⅱ型呼吸衰竭的患者随机分为两组。对照组29例采用常规药物治疗措施;无创组29例在此基础上经口鼻面罩给予BiPAP无创通气。比较两组的临床疗效及治疗前、治疗24h和治疗72h的动脉血气指标、呼吸频率以及心率变化。结果无创组临床有效率显著高于对照组(P〈0.05);无创组治疗24h和72h时的呼吸频率及心率显著低于对照组(P〈0.05);无创组治疗后的pH值、PaCO:以及PaO:较治疗前均有所改善(P〈0.05),且改善程度显著大于对照组(P〈0.05)。结论无创通气辅助治疗肺心病合并Ⅱ型呼吸衰竭能较快改善动脉血气,提高临床治疗效果。
Objective To investigate the clinical effects of non-invasive ventilation technology in the adjuvant treatment of pulmonary heart disease complicated by type I1 respiratory failure. Methods Fifty-eight patients with chronic pulmonary heart disease associated with type I1 respiratory failure patients from July 2010 to October 2012 in our hospital were randomly divided into two groups. The 29 patients of control group were given conventional drug treatment; and 29 patients of noninvasive group were given BiPAP noninvasive ventilation on the basis of nose and mouth mask. The clinical efficacy and arterial blood gas analysis, respiratory rate and heart rate changes before treatment and 24h and 72 h after treatment were compared. Results The clinical efficiency rate in the noninvasive group was significantly higher than control group (P 〈 0.05). The breathing frequency and heart rate in the invasive group 24 h and 72 h after treatment were significantly lower than the control group (P 〈 0.05). The pH value PaCOz and PaO2 in the invasive group after treatment had improved (P 〈 0.05), and the degree of improvement was significantly greater than the control group (P 〈 0.05). Conclusion Noninvasive ventilation adjuvant in the treatment of pulmonary heart disease complicated by type II respiratory failure can quickly improve arterial blood gases, and improve the clinical therapeutic effect.
