摘要
目的:了解我院细菌耐药性变化与氟喹诺酮类药物用药频度(DDDs)之间的相关性。方法:回顾性调查2004-2011年我院氟喹诺酮类抗菌药物(环丙沙星、氧氟沙星、左氧氟沙星)的DDDs及同期细菌(大肠埃希菌、铜绿假单胞菌、肺炎克雷伯菌)耐药率变化,采用SPSS11.5分析二者之间的关系。结果:环丙沙星DDDs与大肠埃希菌耐药率负相关(r=-0.847,P=0.033),氟喹诺酮类DDDs与铜绿假单胞菌对左氧氟沙星耐药率正相关(r=0.958,P=0.003)。结论:氟喹诺酮类药物DDDs变化与细菌耐药率之间关系复杂,同类药品的使用可影响细菌对其耐药率;细菌对氟喹诺酮类药物耐药率整体呈上升趋势,在DDDs降低后,可出现滞后性的缓慢下降。
Objective: To analyze the relationship between the DDDs of fluoroquinolones and the bacterial resistance in our hospital. Methods: A retrospective study was conducted using SPSS 11.5 on the correlations of the DDDs of fluoroquinolones (such as ciprofloxacin, ofloxacin and levofloxacin) and the changes of resistance rates of bacteria (such as Escherichia coli, Pseudomonas aeruginosa and Klebsiella pneumonioe) during 2004- 2011 in our hospital. Results: There was a significant negative correlation between the DDDs of ciprofloxacin and the resistance rate of Escherichia coli (r = --0.847, P = 0.033 ) and a significant positive correlation between the DDDs of fluoroquinolones and the resistance rate of Pseudomonas aeruginosa to levofloxacin (r = 0.958, P = 0.003 ). Conclusion: There was a complex relationship between the DDDs of fluoroquinolones and the bacterial resistance rate which may be affected by the use of drugs of the same class. On the whole the bacterial resistant rate to fluoroquinolones was on the rise, and the bacterial resistance was delayed and declined slowly when the DDDs decreased.
